New from me in @unherd: When gender medicine crashed out Secret WPATH videos open a woke time capsule My very presence here as an indigenous person contributes to and reinforces settler colonialism.” So declared Charlie Amáyá Scott, a presenter at a gathering a few years ago of the World Professional Association for Transgender Health, or WPATH. Yet despite fears about reinforcing colonialism, Scott touted WPATH’s larger mission: the promotion of modern Western gender medicine to address the traditional “two-spirit” identities prevalent among some traditional Native American communities. Scott’s presentation came from a batch of nearly 500 videos of conferences held by WPATH that I recently obtained. It’s a motherlode of material on pediatric gender care that the secretive activist medical organization had long fought to keep from the public. The tapes, subpoenaed by the Alabama attorney general in response to a federal lawsuit, covered a pivotal period from 2021 to 2023, when pediatric gender medicine first began to face political backlash. In addition to egregious lapses in judgment on the part of WPATH, the videos are a case study in the excesses of peak woke, neatly preserved, as if in a time capsule.  During a long winter of dyspepsia, I digested well more than 100 hours of the academic faithful checking their privilege and preaching about “centering lived experience.” And I can now report that there are few subcultures more slavishly faithful to the woke religion than WPATH was during the post-Racial Reckoning micro-era.  Link: unherd.com/2026/04/when-g…

The label has been used to shut down scientific inquiry, intimidate academics and clinicians, mislead the gender public, and foreclose thinking. He wasn't listening to a word I said, so I asked the study's senior author detailed questions about the paper, as well as a Swedish expert on the subject, and published their responses in this Q&A. The Adolescent and Young Adult Gender Dysphoria Outcomes Study (AYAGDOS) is still open and seeking volunteers for a research study about youth with gender dysphoria. Both gender dysphoric youth (13-25 years of age) and parents of gender dysphoric youth of those ages are eligible. The Utah review is a goldmine for anyone looking to learn how not to conduct a systematic review. You can open it to any page and spot mistakes (I am not exaggerating).

After Utah passed a moratorium on medical youth gender transitions in 2023, activist groups lobbied for an evidence review to prove this practice is safe and effective. The result was the "Utah Review," which produced the desired answer by scrapping methodological standards. /1

The two years following Chase Strangio’s 2021 address were a period in which statehouse Republicans escalated their attacks on this field. The WPATH conference presenters largely responded to the political siege by doubling down. Rather than engage in soul searching over whether their methods in pediatrics were ethically sound and whether any criticisms had merit, they overwhelmingly stuck to their guns. Presenters frequently downplayed fundamental hazards about irrevocably altering adolescents’ bodies. Meanwhile, a parade of systematic reviews—the gold standard of scientific evidence—was concluding that the evidencebacking pediatric gender medicine is weak and inconclusive. These findings have led health authorities in a number of European nations, concerned about risks such as infertility, to reverse course. They reclassified pediatric gender-transition interventions as experimental and sharply restricted minors’ access. Not WPATH.
