A rare case where I disagree with @acoyne on policy. His analysis oddly assumes that such a significant change to provincial policy will have static rather than dynamic effects. I concede that the dynamic effects could involve tradeoffs that need to be managed. But notion that it will leave everything else—including the hours worked per physician, or the number of physicians, or how physicians organize themselves—unchanged is rather flawed. In the short term, it will almost certainly mean some doctors work more which will be additive to the province’s surgical output. Over the medium term, it could involve the government loosening or eliminating the cap on medical-school seats at the University of Alberta and the University of Calgary as well as an influx of doctors from elsewhere in response to the market incentives such that the overall supply of physicians is likely to grow. And over the long term, we should see new business arrangements—including a rise in private capital into operating spaces and technology—that should further boost Alberta’s overall surgical capacity. There may more or different reforms for the Alberta government to consider. But the idea that its dual-practice policy won’t have positive-sum effects on the number of surgeries performed in the province strikes me as conceptually “dodgy”.
