Supine Rt. UMPCNL 9.5Fr on a 7 mo child. Two stones- Pelvis 1 cm +middle calyx 8 mm. What treatment option would’ve you offered?

Patient with 17 Left stones. Used @RetroPerc for access. Under 15 minutes from start to sheath in. Case done in 1 hour. Stone Free!

@kjdelay1 I typically will after aspirating and decompressing the collecting system. Minimal diluted contrast. Your 5Fr is already up there. May as well see the pelvis and be sure.

Infected stone. Calculus visible on fluorp Retrograde pyelogram yes or no?

@Tom_Marcelissen I use it rarely (1 every couple yrs). When there is an extremely large prostate or a large tumor in the bladder neck where the UO is only visible with a 70° lens when trying to stent. Open-ended, angled glide with the alberan's bridge is very helpful.

@txsportsdoc Here's Medicare urology reimbursement over the past 3 years. Note radical nephrectomy (3rd from right) and radical prostatectomy (left of middle). It's untenable..

In 1990 srgns under Medicare were paid avg $3,258 for a knee replacement today avg $1098. Considering inflation with no increase surgeons should be paid $7,656 today yet our expenses have gone up 2.35x,hospitals cont to receive ⬆️ reimbursement.

@jpavs17 Yes. Some of the worst post radiation urinary symptoms I have seen have been after proton beam radiation. Most seem to resolve with time. But, sometimes it's many months later.

Another day, another prostate cancer patient post-proton beam therapy w/urethral obliteration. I know good outcomes are out there, and it can happen with any form of radiation. I cringe when patients seek this out due to “less side effects” 😬 Anyone else feel the same?

In high school and college, I was on debate teams in which we learned how to defend *both* sides of every political issue. I can't imagine what debating clubs must be like today--now that only one side is accepted as legitimate. Do debate clubs even still exist?
