@Factor_XII Didn’t have it back yet. Tapped the knee before blood was drawn by nurses.

EM people: who’s calling ortho based on the gross look of this knee tap, and who’s waiting for the cell count and gram stain? Ortho people: you want to be called before of after the lab results?

@luisfgiraldo Sounds like it should be tested in a randomized clinical trial. That would be something.

@TheSurajKannan @soupvector @IdVilchez I think the other thing that irks some of us is the studies that favor fidax for recurrence tend to have someone with a COI with merck. The opt 80 stopped at 4 weeks for recurrence. I saw a positive article today in jid for fidax by the same group.

@soupvector @MBrigmon @IdVilchez A bit surprised by that - didnt that opt-80 trial (or whatever the name was) show lower rate of recurrence with fidax over vanc? I figured the reason we favor vanc (even extended vanc regimens) over fidax at jhh was cost.

I was informed today by my surgical colleagues that their boards state that fidaxomicin is better than vancomycin for c diff. Is that a consensus in the ID community?

I prescribe ivermectin more than most doctors. Because I am an infectious diseases specialist so I use it for a range of parasitic infections in patients referred to me. I never use it to treat cancer because a) I'm not an oncologist and b) it doesn't treat cancer

@DrToddLee @StevenMathern @MBrigmon @costplusdrugs Depends on what is meant by "better". Fewer relapses if used UP FRONT (no evidence after prior courses of Tx). But the NNT to prevent a relapse is 10. And no improvement in up front cure or mortality. As Dr. Lee says, current pricing doesn't make sense given this NNT.

A thousand replies and not one human study showing that ivermectin treats cancer. Zero. Not even a small case series. The published evidence is mostly in vitro studies where cancer cells are killed by a dose of ivermectin that would also kill humans. Not looking promising…
Nick Mark MD@nickmmark

Hey ivermectin cures cancer folks: I’d love to see this “evidence” you keep talking about Do you mind sharing the best article in support of that? Two things: 1️⃣Post the link 2️⃣say whether the study was performed in cell culture, animal model, or humans. Thanks.

@nickmmark The timeline explanation made it sound like most ai experts thought no one knows when but probably longer than you think.

Was listening to a podcast where two AI optimists were debating the likelihood that an AI has built a Dyson sphere by 2030. One suggested a 15% chance!?! Another thought that was too low!?!? These are *insane* estimates that seem like a recipe for certain disappointment..
