@AdamMeakins I think a lot of the time it’s playing with mindset. Using what the athlete believes helps to get them through the events. If that’s a bit of sham therapy then it’s all good 👍 then proper rehab post event.

@CorKinetic If someone is active but not moving well due to fear of pain. Would it not benefit to provide exercises to improve or normalise functional movement. For example when someone protects the back and keeps it straight using hips and knees more to enable them to keep active.

@AdamMeakins But what happens if a patients belief is that them modalities work and help their pain. Use what works for the individual along side what we know or the evidence suggests. No treatment is better than the other if it works for the individual use it as a way in.

@VEstrerg @CorKinetic I agree. If we move with fear and protection and not actually move the back. Then we will see more pathology and more flare ups more chronic pain. If we reduce fear and normalise movement then we start to continue with resistance and functional exercise. But if we just move?

The journey should start with how can I understand your journey so far and how can I help you to progress. I believe in using treatments as a gate way to making connection rapport and trust building. That’s why I use hands on manual therapy along with the exercise prescription.

When the nervous system is in a reactive fight or flight state most protective responses are amplified. Making a mild MSK knee pain jump from a 4/10 to a 10/10. This pain experience then drives fear and disability along with catastrophic thoughts.

MSK pain: can we fix it without actually understanding the person who stands in front of you. The bigger picture provided by a good thorough empathic examination approach enables us hear why the patient needs help. We can’t always fix pain with modalities or specific exercises.

@jackconran8 @AdamMeakins As long as the energy demands of the body is met and recovery is maintained. Good nutrition, hydration and awesome sleep and continued stretching and relaxation. General increase in activity levels making sure we focus on time we are more inactive rather than the time we are.

@jackconran8 @AdamMeakins After an injury we all feel discomfort and pain(normal)some more than others all depending general sensitivity of the nervous system at that moment in time. If some is in a protective state of fight or flight worrying thoughts of pain related to damage this needs to be addressed.

@jackconran8 @AdamMeakins Once a patient understands that graded loads of 10% increase a week over a period of 3-6 months with 3 x a week of consistent exposure we will see good progression in the ability of the tissue to respond well to load. Tissue will adapt and grow.

@jackconran8 @AdamMeakins Once fear of damage and sensitivity pain is more understood through education and that pain of 4-5/10 when exercising is ok as long as it reduces after and ok next morning. Having a rest day then go again chasing pain. Pain is good it protects us and lets us know we are alive.
