I’m at the point where I simply want to cry about this. I’m using the toxic tar shampoo, taking expensive pro-biotics, drinking homemade bone broth on an empty stomach, putting hydrocortisone cream on it….and still it’s there. All the time. What am I supposed to do???

@noahkaufmanmd @Medical_Nemesis Crazy question. Will ambulances bring patients to you? Or are they contractually bound to only take patients to the area’s hospital associated EDs?

Since I am a very experienced, board certified ER doc, I can “handle” almost anything. We don’t have stat MRI nor CT nor troponin (yet) so those show you where we are limited initially. I’d say we can do 90% of what I do in the ER safely. But need to discuss risk with patients. For example, we can treat a kidney stone, and make sure urine not infected, and I can US to look at hydro, but can’t delineate stone size and potential need for Uro. But can do far more than an avg urgent care up to that point. We can do hospice care. Major injuries and dislocations of any joint. Pain control. Procedures. Etc

For over 20 years now I thought that I had to work from the hospital as an emergency room physician. I needed someone to buy me the medications, and give me the space and buy the needles and all the gear. Now I see that was just an illusion and I can do it all on my own.

Your doctor didn’t sell his practice because he wanted to. He sold it because the hospital next door was getting paid 5x more, by Medicare, for the same procedure. Medicare calls it a site-of-service differential. Economists call it a market distortion. Patients call it a $400 copay for a blood draw.
