The accusations against Swalwell aren't he-said-she-said. They're well reported, fairly corroborated, detailed and damning. And while he's denying them, it's with precise language ("These allegations are false" v. I have never engaged in any such type of behavior). He has allies abandoning him (Pelosi giving the TKO) and, in all likelihood, more stories coming. This is unsustainable.

I’m from Sweden and also a social democrat (Socialdemokraterna). And I’m getting tired of the left fucking eating it self from the inside. The right is somewhat like in the US now unified in Sweden. And the other parties on the right have now also accepted to include the far right party Sverigedemokraterna to be included in the government if they as a coalition get majority in the upcoming election. And the left; what are they doing? Absolutely nothing, they can’t unify about anything it seems. They are torn between ”small” issues and wanting far to much say for their involvement in a possible government coalition. Sweden is soon gonna turn from being a beacon of equality to a far-right cesspool if they win this election. The real reasonable left need to stop supporting these absolute good for fucking nothing socialist and communists, that do nothing but create division and dissent in the left. I feel like when these people get the promoted views from the left, all they do is opening the possibility for them to steal votes and people that could be otherwise contributing to a more healthy left, instead of just wanting to burn everything down.

@SethMoldenhauer @IM_Crit_ The most important thing to keep in mind is that the LV is empty not because of global hypovolemia, but because the RV is unable to pump enough to fill the LV. The massively dilated IVC tells you there is enough volume- you just need inotropes to get it to where it needs to go

I see where you're coming from and that was my initial thought also, but then I considered the vasodilation of the inotropes. Also my understanding (I'm just a nurse), is they have far more effect on the LV than the RV, so wouldn't that have the potential to worsen LV filling 2/2 to increased SV and decreased filling? Or will the inotrop work on the RV enough? Obviously cath +/- RP Impella is the end point, but I was trying to stay true to the question. @CoffeeBlackMD, curious about your thoughts on this. Just trying to learn. I appreciate the convo 🤝

@SethMoldenhauer @IM_Crit_ Dobutamine does not typically cause systemic vasolidation or lower the MAP, especially if cardiac output is low. It can cause reflex pulm vasodilation, which is beneficial here. The RV is as responsive to inotropes as the LV.

500 bolus. Right sided failure post PCI. So RV function is preload dependent. Even though it seems like there is elevated pressures on the right and IVC is dilated, we still need preload to force fluid through. Lactic is elevated due to decreased CO secondary to right sided failure and inadequate filling of LV. Dobi could help a little, but could potentially worsen the issue if the LV is empty. RV after load is not the issue so pulm dialator doesn't help much. Nitro, lol.

Look, the dumbest thing about all this is that I didn’t and don’t care about Piker. Piker never comes up in focus groups with voters as someone they’re following—the way a Rogan or Tucker or Candance does. I don’t think he’s singularly making an impact on politics in any way commensurate with the attention he’s getting. But I care about Tim. I agreed with @Timodc’s original point that using Piker as a litmus test is dumb. Calling up Jon Ossoff to ask what he thinks about Piker is dumb. It is an unnecessary distraction. But I felt like Tim’s discussion of Piker was too dismissive of some of Pikers more toxic positions. And that was the part I wanted to react to. There’s nothing I can do about the fact that the internet distortion field has overtaken the whole thing. And that people are debating my views in a way that is totally divorced from my actual, very-well documented views. That’s just part of what happens when doing this work. The only thing I can do is keep saying what I actually believe. And what I hear voters saying they believe. But politicians can reach voters who are angry about the Iran war and even about America’s relationship to Israel without going on the stream of a guy who said America deserved 9/11. But, it’s also no doubt true that he’s being elevated by the whole conversation and the right answer to questions about him should probably be “Piker, who?” Because that’s what most voters would say if you asked them.

Buddy, I don't know what to tell you accept i was 'comfortable' with Liz Cheney because she (correctly) identified that Trump was a fundamental threat to our American society and the world, and did not make any ideological demands for her endorsement. Despite being a lifelong conservative, it was *that* serious of a threat. And she was right... The major difference is that Hasan, not only did nothing to help defeat Donald Trump, but he spent the election shitting on Dems and demotivating voters. He constantly does this. He is a wrecker. He wants to see the Dems lose (unless they agree with him 100%) and he does his best to help make it happen.

If this is an honest question, I’d say: Americans are rooting against America because we facilitated a genocide and followed it with a surprise attack on a girls elementary school followed by attacks on universities, medical centers, more schools, a world famous pharmaceutical research center, a volley ball team, an unfinished bridge we claimed was transporting weapons and then a nuclear power plant. We are now promising endless attacks on civilian infrastructure. We are hunting and targeting anyone who might be involved in ceasefire negotiations. Most people do not pay enough attention to have absorbed all the propaganda about the U.S. and Iran. So people coming to this fresh see us for what we are: absolute monsters. And monsters must be stopped. That’s why people are rooting against us and for civilization to prevail.
