#FromThenToNow, our #SCN has largely grown, now more than 6x the number of partner locations across our footprint than our start! Together, we've been screening for + providing services to address #HRSNs in our communities; we're at the forefront of change for health of NYers!

The core goals include strengthening public health data, accelerating access to analytic solutions, visualizing insights for action, and advancing interoperable data. This strategy acknowledges the critical role of social determinants of health (SDOH) in shaping health outcomes.

Last week, the @cdc released their 2024-2025 Public Health Data Strategy which aims to strengthen public health data infrastructure, reduce data complexity, promote health equity, provide timely data, and improve health outcomes through four core goals!
