Sadly, this is not a bug. It is a feature and, in many ways, the intended outcome of the policies now in place. Stories like those of these two physicians will keep repeating in the months ahead. To put it simple: if those two physicians had not been born in Venezuela, but instead in Sweden, Turkey, or the Philippines, Dr. Bolívar’s green card application likely would have been approved months ago, assuming she had maintained status at the time of filing, and none of this would have happened. A similar pattern played out with Dr. Véliz, whose program filed for a J-1 visa on his behalf, yet his case remained in limbo for five months without either an approval or a denial due to the pause, ultimately costing him his job and career. Recent reporting and public accounts describe exactly these kinds of outcomes. Let me explain why. On December 2, 2025, and January 1, 2026, USCIS issued PM-602-0192 and PM-602-0194, creating adjudication holds tied to applicants from designated “high-risk countries.” This is not the same as a travel ban at the border. It affects people already living inside the United States, including physicians who are already here, already trained, and already treating patients. In practice, that means a legal immigrant or physicians from those countries can timely file through an employer, a U.S. citizen spouse, or on their own behalf, and still receive no decision indefinitely. The hold will leave people stuck, with the factor of time, underlying status will expire, employment collapses, and their legal footing erodes through no fault of their own. Being in authorized stay comes with risk of detention with any encounter with ICE during this administration despite following immigration laws by the book. The logic is as cruel as it is simple: delay the case, let status lapse, then punish the person trapped by the government’s own delay. That is why this policy is so dangerous. It turns administrative limbo into a pipeline for exclusion, job loss, and, eventually, detention or forced departure based solely on country of birth. It is an effective but subtle way to increase deportation numbers from disfavored groups already inside the country: mass delegalization through delay, followed by coerced self-deportation after blocking the pathways needed to remain employed or extend lawful status. Physician drew attention to this policy because even the loss of a single physician can have an outsized impact on a community.
