New therapeutic options don’t come along often in palliative care. When they do there is often understandable hesitancy to introduce these into practice. Clonidine is a good example of this so I wanted to explore more with this thread 🧵 👇

Opioid switching - A thread 🧵 Not all opioids are the same- that’s where we start this thread. Morphine is the archetypal opioid and best known but there are a multitude of other options whose use and availability varies across the globe.

I had experience of this earlier in the week when our baby daughter was born at St John’s. A moment I will cherish as a dad forever, what a brilliant person centred idea 👏should be made available everywhere

Did you miss our midwives on STV News last night❓ You can rewatch the incredible news piece on Cozy Cuddle Scrubs (by Stacey)™, designed to help birth partners provide immediate skin-to-skin contact with their newborns 💕 Watch it here: ow.ly/ja9n50UlC0i

Nadhim Zahawi has agreed to pay the £3,000,000 tax bill he lied about and hid from HMRC. He should be sent to jail for fraud. RT if you want to see him in jail.

Domperidone, QT interval & prucalopride: a🧵 Prucalopride is now off patent and its price beginning to fall At the same time, evidence of a gastroprokinetic effect suggests it acts throughout GI tract, not just on the bowel... cureus.com/articles/21767… cureus.com/articles/55806…

@HarrisElayne @revealshadow @griffiths_gill That’s good to know Elaine, I don’t understand it from a car mechanic background. One pushes (drives) plunger the others draw the fluid through using downstream negative pressure. I wonder if patients/families have different reactions to the names

Time to revive this thread 🧵 Syringe driver doesn’t = dying Dying does not mean a syringe driver is needed Many of the same medications commonly used just given by a different route
Tony Duffy@Existential_Doc

Syringe drivers: A palliative care thread Let’s cover a topic that can be very emotive and frequently misrepresented Administration of medications via continuous subcutaneous infusion (CSCI) First lets cover some common concerns we may encounter when considering using a CSCI

Both used across all sites ive worked in before. I tend to use driver as it makes techincal sense to me. When introducing to people ill talk over how the device works (small fixed movements of the plunger) over each hour and delivers the medication under the skin as opposed to into a vein. Always talk about steady state delay but flexible dosing after that.

Rishi Sunak has decided that the £14.9bn of PPE fraud cannot be investigated & has written it all off This is the same amount of money as the entire budget for General Practice & Primary care for a year Imagine writing off 330million consultations, GPs, Nurses etc for a year

@Existential_Doc Couldn't agree more. So many upper GI cases just now it seems. Very complex patients being treated at home

It feels like there has been an sustained rise in the need for complex end of life care. Am I imagining that there has been a trend towards Particularly aggressive presentations of cancers esp cholangiocarcinoma, pancreatic, oesophageal, myeloma and rarer lung cancer subtypes? Often in young people.

DC_Police Dogs ( Puppy Development Program)
There is a reason why the German Shepherd has been the go to dog for the last 70 years. But I believe Giant Schnauzers like PD Albert & TPD Bert have the qualities to supplement our current breeds the GSD, Malinois and Dutch herder. Great day with the boys and ITV yesterday
