@EdinburghKnee @NHS_Lothian Right with you sister - I’m waiting for a similar lump sum for missed oncall payments since I started 18m ago. 🤦🏻‍♀️

Meniscus root tears are important to recognize and address. Unfortunately, the natural history of a medial meniscus root tear can be seen in this patient when left untreated, with rapid progression of medial arthritis. #savethemeniscus #orthotwitter

2022 has, frankly, been a total sh*t storm. Despite this, I am so delighted that, against all odds, myself and my husband have secured consultant jobs within a month of each other I could not be more excited to start my Paediatric/Knee journey with @SotonChildHosp and @PHU_NHS

@bellkneesurgeon Great thread, thanks. the thing I have taken home about these entrapped gutter fragments is that they often give a more ‘arthritic’ type symptom pattern with constant pain rather than the more common intermittent sharp pains on twisting and deep flexion seen from standard tears.

After resection what I consider a good prognostic sign. There is synovitis ( redness) on the capsule adjacent to where the meniscus has been causing pain.might pain and symptoms tend to settle very rapidly in these patients.

Is arthroscopy the right decision? 2M symptoms. Medial meniscal tear - see the fragment in the medial recess tucked in between the MCL and the tibia. There is secondary bone oedema from the pressure of the fragment on the bone. N/1

What happens when the meniscus gets detached from the bone? The meniscus no longer works as a shock absorber as it is pushed away from the joint (extrusion). This is why it is important to repair Root Tears #savethemeniscus

@deboraheastwood It is happening everywhere. Eg calls to potential candidates’ home deaneries and trainers asking what their home situation is, if they have young kids, what their partners do. The inference is if you have young kids and a working partner you’ll be less invested in their job. 🙄

The leaky orthopaedic surgical training pathway for women… I’ve often wondered what the rate of drop off is at fellowship level? Most fellowship jobs are only offered as fulltime and I have witnessed a lot of bias against employing female fellows with young children. 🧐

@MariThiart Not my data! I suspect it’s from last year as there are now a couple of women on the bscos board appointed in 2021…

@TomWalshaw Fellowship can be a brilliant opportunity to bring new techniques, skills and perspectives back home, I don’t dispute that. I just wonder if some nationally sponsored posts with a national recruitment process like they have for the TIGS might help make things a bit fairer.

@TomWalshaw Certainly true that orthopaedic training in other countries can be done very differently, I understand speciality training starts very soon after medical school in some countries e.g. New Zealand which must surely help streamline training.
