Procedural aspects of prayer and fasting etc are usually overkill. Yes we need to know zakat rates, but Too much micromanagement has lost the forest for the trees.

This is indeed a serious crisis, and it’s even worse to see it take shape irl within our communities. We need to do better at engaging with youth intellectually and socially and inviting them to the masjid! It requires sincere effort and creativity!

Vit E taken in long term isn’t an issue. Pentoxy is less well tolerated. I’ve been recommending prophylactic Boswellia for a year without adherence issues. Again, I think starting these near term in the absence of symptoms is the way to go. But I’d welcome a study testing this hypothesis. Would be a great trial for NRG HN group :) @sueyom

@NiuSanford Just a comment on bev. I saw a patient treated elsewhere that got a tracheoesopahgeal fistula from 30/10 while on bev. It’s the only reason I could see it happening. Anytime I do anything for patients with bev I try to washout for weeks.

Longer post. For example, 2 v senior, thoughtful rad oncs I spoke with: 1 continues to hold cytotoxic chemo 2 weeks before & after any RT, the rational being this is what he was trained to do. On other end of spectrum, the other rad onc holds bev only on day of lung SBRT (no washout before), even for central cases. So, wide practice variations and of course there will be case reports of severe toxicity but we don’t know denominator or even counterfactual. Radiosensitization vs. additive toxicity are really two sides of same coin. Combined toxicity is real, but I do wonder if we are missing opportunities for therapeutic synergy, stopping systemic therapy unnecessarily, or withholding RT when it could be beneficial, “out of an abundance of caution.” My table reflects this perspective.
