Beyond glucose and weight loss: GLP-1 RAs reduce systemic inflammation, atherosclerosis inflammation and progression, and MACE —independent of diabetes or obesity Key findings: * Using serial intravascular NIRF-OCT inflammation and IVUS imaging, liraglutide reduced plaque inflammation and plaque progression in rabbits without obesity or diabetes; * In the MGB Biobank, GLP-1 RA use reduced blood inflammation and MACE, regardless of A1c or BMI status; * In human macrophages, three GLP-1RAs suppressed inflammatory cathepsin protease activity; --> Intravascular NIRF-OCT offers the potential for inflammation imaging and anti-inflammatory drug assessment in human CAD --> Time to investigate GLP-1RAs in all patients with CAD? New from @rayshafarah in our group published in @JACCJournals 🔸 A single measurement of 4 blood-based biomarkers in midlife, namely CAD PRS, hsCRP, LDL-C, and Lp(a), is strongly predictive of future CAD incidence in both sexes and across the age spectrum from 40 to 69 years 🔸 A biomarker-based prediction model achieves comparable performance with clinical risk calculators and may identify populations who are not detected using a traditional clinical risk calculator, creating an opportunity for improved primary prevention, particularly in younger age groups, where the performance of biomarker-based screening is highest. 🔸 Interpreting risk as a cumulative burden of genetic, lipid, and inflammatory pathways provides a straightforward approach to prioritizing earlier, targeted primary prevention. New Review Article: The Inherited Basis of Coronary Artery Disease 👉 nej.md/4tmGZtI Coronary artery disease develops from an interplay of behavioral, environmental, genetic, and stochastic factors. Smoking, hypertension, hyperlipidemia, and diabetes mellitus are well-recognized modifiable risk factors for coronary artery disease, which affects almost 1 in 2 men and 1 in 3 women during their lifetime. More than a century ago, Osler posited that inherited factors may cause coronary artery disease after he observed that angina pectoris often clusters in families. Approximately 30 years ago, twin studies indicated that the heritability of fatal coronary artery disease was as high as 50%. Since 2007, large-scale genotyping and gene-sequencing studies have identified hundreds of genetic variants associated with increased susceptibility to coronary artery disease (seen in figure). Protein products of some associated genes already constitute effective treatment targets, and other genes point to currently unexplored disease mechanisms. An inquiry about a family history of coronary artery disease is routine in the clinical assessment of patients presenting with chest pain syndromes.
