The OPD was running warmer than usual that afternoon. Fans spun with tired persistence. A queue moved in small, patient increments. Files carried stories waiting for translation. This was the hr of fevers. Viral fevers. Dengue. Typhoid. TB. Dr. X glanced at the next file. “Fever, 3 weeks.” He looked up. The man entering didn’t look 'fever' . No flushed cheeks. No fatigue in the shoulders. No slow steps. No shawl wrapped against imaginary chills. He walked steadily. Sat comfortably. Smiled politely. “I keep getting fever in the evenings, doctor,” he said. “How high?” Dr. X asked. “Not very high. Around 100… sometimes 101.” “How long?” “Three weeks.” “Chills?” “No.” “Body pain?” “No.” “Cough?” “No.” “Burning urine?” “No.” The students relaxed slightly. Viral fever. Maybe enteric. Maybe nothing. Dr. X did not write. “Any back pain?” “Not really pain… just discomfort sometimes. Right side. Dull. Comes and goes.” “How long?” The man thought carefully. “Maybe… two months.” Silence entered the cubicle. “Any weight loss?” “Yes,” he said immediately. “I thought because of fever.” “How much?” “Five kilos.” Dr. X leaned forward. “Do you sweat at night?” “No.” “Feel weak?” “No.” “Feel sick?” “No dr. That’s the strange part. I feel normal.” Dr. X nodded slowly. He palpated the abdomen gently. Left side. Soft. Epigastrium. Normal. Right flank. He paused. Pressed deeper. A fullness. Not tender. Not painful. Just present. Instead, Dr. X wrote quietly: USG Abdomen The students were confused. “Sir… fever workup?” one whispered. Dr. X shook his head. “Fever of unknown origin is a diagnosis of patience,” he said. “Not assumption.” The report arrived the next day. Large heterogenous mass- right kidney. The cubicle was silent. CT scan followed. Enhancing renal mass. Suspicious for Renal Cell Carcinoma( Kidney Cancer). The students stared at the screen. “But sir… fever?” one asked softly. Dr. X nodded. “That is the lesson.” He turned toward them. “Renal Cell Carcinoma is a master of disguise.” “It is not just a kidney disease.” “It is a systemic storyteller.” He began walking slowly as he spoke. “The tumor releases cytokines, especially IL-6.” “IL-6 resets the hypothalamic thermostat.” “Evening fever appears.” “Not infection.” “Inflammation.” He continued. “Tumor cells also produce prostaglandins.” “Result, fever without chills.” “No rigors.” “No toxicity.” “Because there is no pathogen.” “RCC can produce erythropoietin, causing polycythemia.” “Renin, causing hypertension.” “PTH-related peptide, causing hypercalcemia.” “ACTH, causing Cushing’s.” “Paraneoplastic syndromes.” “Signals from a silent organ.” He paused. “And pain?” a student asked. Dr. X shook his head. “The kidney lives in space.” “Tumors grow outward.” “No early obstruction.” “No early pain.” “No early symptoms.” He looked at them. Silence settled again. “Fever,” he said softly, “may be the first whisper.” Later, in the counselling room, the patient listened quietly. “Is it curable, doctor?” he asked. Dr. X answered gently. “We have treatment options.” When the door closed, the students asked the question they had been holding. “How did you suspect kidney before infection?” Dr. X closed the file. “Because infection makes people feel sick.” “Cancer often does not.” He looked toward the waiting room. “Fever alarms the patient.” “But weight loss…” he paused, “whispers to the doctor.” The fans continued their slow circles. And Dr. X waited for the next pt to walk in.

₹300 paid. 60 minutes wait. Opened the box, rotten smell from the mushrooms and even the rice. Entire Mushroom Pulao went straight into the dustbin. And the real shock? No proper complaint channel. No human support. Only options: take ₹80 coupon/refund or “share feedback” and close the case. So customers either accept peanuts or stay silent? @zomato this is not service, this is escaping responsibility. You take full payment, charge platform & delivery fees, but won’t ensure basic food safety or allow customers to actually talk to support. @deepigoyal, food quality at partner restaurants is not being monitored and genuine complaints are being pushed away. Customers are not feedback forms, and unsafe food is not a small issue. Fix your quality checks and customer support immediately.
