@ladydatadoc @SBlake2000 @realdocspeaks It’s called there maintenance choice plan. (Or lack there of.) You have to be a Cleveland Clinic patient I think but it’s outpatient and works exactly like a retail pharmacy on my side since I am one.

@ladydatadoc @realdocspeaks Then let's agree that most of the drugs that are generic and thus too cheap to insure should be removed from insurance coverage entirely, which means that those pesky PBMs -- which let's face it, are abusive and mostly unnecessary -- can focus on the "new and expensive drugs."

@DrMTd8e @CVSHealth Humalog (or Lispro I don’t care). I’ve been on Humalog for 30 years. I am deathly allergic to all insulins made by Novo Nordisk, and such allergy is well-documented in my medical file. They require the PA because they want me on Novolog.

@SBlake2000 @zonefive8 @realdocspeaks I have no idea how that’s sustainable. Dental insurance pays Dentists even less if you can believe it! I really like Epic/My doctors so I just decided to eat it and pay the bills to go to them. (I could say go to MetroHealth it’s a little cheaper I think.)

@chip678 @zonefive8 @realdocspeaks Yes. Those are my hospital system billing prices too. But at another independent doctors office I go to, I pay a $55 co-pay and my insurance pays him $19. That is $74 he gets for the visit, amd he has to pay overhead on that!

@SBlake2000 @zonefive8 @realdocspeaks Well UnitedHealthcare is totally worthless and denies everything. My last visit was only $250 to be fair. (That’s the full non insurance price.) But usually my visits are $400-450 which is the full price. It’s an hospital system yes.

You are feeding the beast with that statement. First of all, it is current taxpayers and people on Medicare through their Part B premiums that are paying for that visit. Secondly, Medicare imposes horrifying burdens on physicians -- notably quality metrics and abusive auditing -- that is burning out your doctor and reducing your access. Further, your Part B premiums would be LOWER if you just paid out of pocket for your visits. It would eliminate burdens on physicians so that they would not have to decide whether to "take Medicare or not" and if you just pay at the point of service you are in control. When the "government" pays they dictate the terms of your visit. Why do you not want to have more power here? Office visits are cheap and predictable. Think about it: the minimum premium for Part B is $202/month this year. Why not have it be $150/month and then each visit is $100 out of your own pocket? You would have to see the doctor six times a year to break even. If you see the doctor less, you are now ahead financially. But more importantly, the doctor can remove all of that Medicare required overhead and you doctor will see you as a paying customer, rather than an irritating billing vessel.
