@mandeep_mayo @maor_elad @PCRonline This was a patient who was brought as an Inferior STEMI, the patient was diaphoretic, SBP 60 mmHg, no radial pulse. Normal coronaries seen, then we injected to the aorta. This was a type A dissection from the get go

@maor_elad @yonigros @PCRonline It is indeed horrifying. Iatrogenic Type A dissection, now involves aortic valve, coronaries&arch. Bad prognosis. Not sure the sequence of events! Coronaries must have been injected through true lumen. My bucket list of complications that I haven’t done/seen is getting smaller

Is there a specific reason in this case to prefer C sec? Regarding post delivery, Melody valve in native RVOT is limited to small MPA, under 22mm. Consider Harmony

The patient is improving with treatment. I would consider continuing it while following kidney function and electrolytes

Where are we with drug-coated balloons (DCB) in PCI? Five thoughts. 1) The use of DCB in patients with in-stent restenosis, especially of a bare metal stent, is well established and evidence-based (now also in the US). 2) Conversely, DCB use in de novo disease lacks compelling clinical trial data. Use in small vessels and diffuse pattern disease is the most promising indication to date. Results of two ambitious trials (SELUTION De Novo AND TRANSFORM II) will define the role of DCBs in de novo disease going forward. 3) Growing experience with short DAPT and DES limits added value of DCB angioplasty in high bleeding risk patients. 4) The good performance of current generation DES in acute coronary syndromes questions the unmet need for this indication. 5) Robust data for DCB use in bifurcation PCI, where the provisional approach is dominant, is lacking.

He made a gesture “100” for the Israelis who have been held hostage by Hamas for the past 100 days. All hell broke loose: - He was condemned by the national football association - His team announced he was suspended - Then they said he’d be fired And to top it up, Turkish police has arrested him (!) and he’s undergoing interrogation. Yes, for making this simple gesture. This is Turkey 2024. Shame on you, Turkish government.
