NEWS: Labour to ration NHS referrals Labour’s plans to ration NHS referrals are deeply concerning and will affect us all. Instead of being referred directly to a specialist when your GP thinks it’s needed, now your doctor will have to email a specialist and you have to wait for them to email back their view. Once your GP gets the email back, they either need to call you in for another appointment, or provide that specialist's assessment to you by letter. Then you will need to wait and see whether that advice (given without seeing you in person) works for a period of time, before then going back to your GP again, and asking again to be referred to a specialist if you are still suffering with the health issue. The result? Longer waiting times for specialist care, while specialists spend more time writing emails than actually seeing patients and using their expertise. On paper, waiting lists will therefore look significantly shorter, but only because fewer people ever make it onto them. You are not on a waiting list while the GP is waiting for the specialist to send their advice (without seeing you). You are only on a waiting list when, and if, you then request later to be seen by the Specialist, once you've gone through all of the process above. ⁠So all of this means the Government can announce they've cut waiting lists massively, but what is actually means is longer waiting times for specialist care for all of us as patients, and specialists spend their time writing emails not seeing patients.

@Jenny_1884 Yup. I worked behind the scenes for one of the emergency services. Shifts covered 24/7, Easter, Christmas & Bank Holidays. A member of public never heard 'we are closed until normal working hours - call back when normal service resumes'. Drs surgeries should do the same.

@Emmerdale12345 @Jenny_1884 Ah the OP spins her usual ragebait wheel & up comes GPs today. There aren’t enough of them vs demand to provide the current service, never mind 7 days a week. Yes, they receive the weighted global sum per pt. If they were paid per appt, they’d be paid more. So now what?

@Jenny_1884 Totally agree. They get paid for the number of patients they have not the hours worked. When somebody misses an appointment the GP still gets paid. It’s the taxpayers money being wasted.

@Damo8375 @matthoade @adamgipki Both feet over yes, body & hands leaning left though & how can you tell, at ground level, in a fraction of a second, from the wrong side, that the whole of the ball is def over whole of the line? The answer is you can’t. A v good advert for goal line tech, but not incompetence.

@Damo8375 @matthoade @adamgipki Talk us through how this is inept? This angle isn’t on the line & you can’t definitively say it’s fully over. Assistant is on the side behind Walker so is having to look ‘through’ him on this. This specific incident is a ‘limitation of the human eye’ issue, not a competence one.

@adamgipki @BaconBantam @I_am_Del_Rio For offsides they stick with line of 2nd last defender. Regardless, I get the frustration but there’s no proof this was 100% over the line. Looks it from images we have, but we don’t know for certain. Assistant is having to look through Walker on this. They can’t guess.

@BaconBantam @I_am_Del_Rio The linesman job is to call for offsides so he’d be in line of the ball by this point (wishful thinking perhaps) but I’m sorry, that’s clearly over the line because the white line is in line with the posts and the keeper is behind it. Regardless, well done on your win.

@I_am_Del_Rio @adamgipki Yep this is basically it. I get Wycombe’s frustration as it looks over the line, but no one has unequivocal proof. Assistant can’t call that with certainty as he’s having to look through Walker, even if he’s on goal line it’s not going to be conclusive given that point.

@adamgipki You would have to be 100% certain that 100% is over the line. This isn’t a refereeing error, it’s human beings managing games without technology. Players accept it and move on. Watching the game, there was no way of confirming.

@ChoppaMo @CunningColin You’re talking to a qualified ref. Look at the guidance for DOGSO - that’s all that matters here. Does this fulfil the criteria? Incidents are rarely identical, so not sure how you can say Trusty red sets precedent. I actually felt yellow given in both was correct before VAR.

@StuartW1976 @CunningColin Clearly you didn’t read what I said. It was more than a SPA which would have been a Yellow, making it an OBVIOUS goal scoring OPPORTUNITY. Key words in bold for you. Precedent was set at Tynecastle when Trusty was sent off for an even less obvious goal scoring opportunity. 🤷‍♂️

Ambulances at A&E's are still waiting anything from 3 hours + to handover care of their patients. A&E waiting rooms overwhelmed by patients with minor ailments because, thanks to Blair, GP's are no longer on call, and the out of hours service, 111, is not fit for purpose.

@ChoppaMo @CunningColin You’ve missed the key word from the laws - obvious. Take a look at the laws & the guidance on judging DOGSO. This really doesn’t fit given foul is near halfway out wide, with the Hearts defender covering back through the middle (who would clearly be able to prevent).

@CunningColin More than a yellow for SPA cos it was more than just a Promising attack. Red for DOGSO would be about right as it was a definite goal scoring OPPORTUNITY. Leonard knew what he had to do and took 1 for the team.

He’s been sent off here for DOGSO, though I’m really struggling with how it is, with the Hearts defender running back through the middle & the foul taking place next to the touchline.

@CunningColin If the defender makes a deliberate, cynical foul—such as holding, pushing, pulling, or pulling the player down without attempting to play the ball—it is a straight red card if it stops a clear, developing counter-attack, regardless of the distance from the goal.

@sturner85 @refsupportuk He’s an experienced international pro. If he’d wanted to avoid GK, he could have (by following initial touch left), or even hurdled him. But he drags right leg into the keeper’s slide to ensure contact. You’re naive if you don’t think he knows exactly what he’s doing.

@StuartW1976 @refsupportuk Again, you're watching this in slow motion. At full speed there's no way he could have avoided that contact. It's 100% not a penalty but it isn't a dive at all. We're setting the bar high if you're including that in your multitude of sins.

@lownyp @refsupportuk Ref (presumably) missed GK’s touch & so yes, it’s an error. But it is also a dive/simulation. Derby player after touching it left drags right foot on ground to initiate contact & is already starting to go over before contact. Subtle, but pros are becoming very ‘skilled’ at this.

@refsupportuk It’s not a penalty .. but it’s not a dive 🤷‍♂️ The keeper got a touch but his momentum took him into the player. If the keeper missed the ball it’s a penalty .. but the refs error is he did get a touch. Why can’t you EVER not be blind to the fact that refs DO make errors at times!

@dogavic @refsupportuk Derby fan sticking up for his own player shocker. I’ve no skin in this game and your player has initiated contact by keeping his right foot sliding on floor into the keeper & then goes over. Call it diving, simulation or whatever. Pros have become v adept at this.

@refsupportuk Grow up, it’s never a dive. I agree the ref gets it wrong bc he misses the fact the keeper gets a touch on the ball, but he then goes on to wipe the player out in midair - what’s he supposed to do? You have no clue what it’s like to play football if you think that’s a dive

But it does nothing for GP capacity & takes GPs away from their appts. Pointless enabling more into the funnel at the front end, but not providing the resource to handle it,

@Wilmslow5kyw @DrHuw Whe does eConsult need to be closed when trying to make Non Urgent appointment ? I eventually made one only to be referred to a physio, then maybe an x-ray, then maybe blood test, then maybe GP then maybe Hospital Appt waiting list for 12 month.

I've been registered with the same surgery for 16 years and in all that time not a single dr there has indicated that they recognise me. Aside from being an inefficient way to practice medicine, I find it really rude! Is this a strategy GPs use elsewhere or unique to my surgery?

@GovernanceGoose @lukejcr There were 14.5m GP specific appts in Jan alone. There is contract-enforced unlimited online triage in place (which brings its own issues, but still). Absolutely would not say the NHS is working well, but suggesting no one can get a GP appt is a massive sweeping generalisation.

@lukejcr In our area, we currently have no real NHS GP service. No one can book an appointment. You call at 8.30am to an engaged tone. The NHS, for us, no longer exists. So I would say to you "what NHS?"
