The importance of ICE can't be understated in any EP procedure. Here you can see the power of #ViewFlex for pap muscle PVCs. @HanyDemo skillfully maneuvers #HDGrid and #TactiFlex to either side of the ant lat pap muscle to map and ablate trigeminal PVCs. #AbbottProud

Successful ablation of this posterior medial papillary PVC! With delivery of RF we induced a flurry followed by suppression. The video in the comment shows post ablation echogenicity, proving lesion formation on the pap! @atrialappendage

At first, the HRA PAC appeared to dissociate the A from the tach, falsely eliminating AT from our diagnosis. However, the HRA PAC did NOT dissociate the A from the tach, but only dissociated the HRA region from the rest of the atria. This observation helped us conclude AT

AT originating from the NCC, just a little too close to the His to burn from the RA. Check out the EGMs that show RA and LA dissociation in tach! PAC from HRA fails to change CS and V activation and timing. Sinus rhythm restored after a few burns to this area💥@atrialappendage

Great LAFL case at UChicago AdventHealth Hinsdale with @ATLawrence. DCCV post PVI induced a 360ms tach. Quickly mapped with HD Grid using OT and found a pinwheel circuit around a small area of scar on PW.

Left Lateral AP mapped in tach using #OpenWindow technique. Who would have thought these unsuspecting EGMs were at the site of the pathway?? Vectors led us right to the spot for a 2nd burn term of this ORT tach. @atrialappendage

Couldn’t afford to induce so we didn’t! EF drop -> 10% after recurrent hypotensive VT > 22 shocks. Refractory to Sot, Prop, Amio, Lido, Proc. Targeted abnormal Purkinjes + functional substrate (#ILAM). Now home >3 months, Metop only w/o rec #EPeeps #AblateVT @atrialappendage

Congenital 2 y/o pt with atriotomy scar from a ventricular septal defect repair. Initially induced Typical AFL that terminated during RF abl where the intrinsic line of block discontinued. Post-testing induced an EAT from the RAA that terminated during first burn! #EPeds #TCSE

Here’s an excellent example of what a strong Workflow Partnership looks like! @MrMapperEP and Dr Bowers had a procedure time of 59min which included treatment of AtypFL, PVI(2 veins open, prev PVI abl), PostWall iso, and CTI. #HDGrid = vital role in procedural efficiency!

Bye-bye redo dual-chamber Atrial Flutter 👋🤓 with a super long CL of 480 ms. A quick map with #HDGrid showed this white area of epicardial insertion! 💥💥💥 and we got it! #epeeps

PAC that initiates AFib in a Patient with history of #Afib ablation x2. Dr O’Neill was able to successfully map and eliminate the PAC and the Pt left in NSR! Mapper @brandon_mukai reported the signal quality and efficiency experienced with #HDGrid was crucial for success!

Educational observation in this post by @subparsonsmaps: because of the countercurrent ventricular wavefront we can appreciate the slant of the pathway, as the PW potential is earliest on D spline and gets progressively later to A spline. Has anyone else ever seen this? #EPeeps
