@TheSurajKannan @soupvector @IdVilchez I think the other thing that irks some of us is the studies that favor fidax for recurrence tend to have someone with a COI with merck. The opt 80 stopped at 4 weeks for recurrence. I saw a positive article today in jid for fidax by the same group.

@soupvector @MBrigmon @IdVilchez A bit surprised by that - didnt that opt-80 trial (or whatever the name was) show lower rate of recurrence with fidax over vanc? I figured the reason we favor vanc (even extended vanc regimens) over fidax at jhh was cost.

@DrToddLee @StevenMathern @MBrigmon @costplusdrugs Depends on what is meant by "better". Fewer relapses if used UP FRONT (no evidence after prior courses of Tx). But the NNT to prevent a relapse is 10. And no improvement in up front cure or mortality. As Dr. Lee says, current pricing doesn't make sense given this NNT.

@DrToddLee @StevenMathern @BradSpellberg @costplusdrugs We don’t have the recurrence rates locally at our institution that others report. For us it is stupid to start someone on fidaxomicin unless they have failed vanc. In most cases i doubt the diagnosis to begin with.

@StevenMathern @MBrigmon @BradSpellberg It is not priced properly. Especially since @costplusdrugs sells Vanco for ~$1 a capsule.
I do suspect fidaxomicin pulse taper is in need of further study. We have new data coming to this space at ESCMID 2025.

A thousand replies and not one human study showing that ivermectin treats cancer. Zero. Not even a small case series.
The published evidence is mostly in vitro studies where cancer cells are killed by a dose of ivermectin that would also kill humans.

I have a patient who is on ertapenem for esbl organism osteomyelitis. @UHC approved 4 wks of antibiotics but now is asking for more out of pocket costs from the patient for the last 2 weeks bc of the new year. Why are you so non compassionate?
