The “Fluid Seeking Phase” is real, and it’s spectacularly important to give that volume over first 12-24 hours… then start the lasix:)

True! Important distinction in the early post operative period. I conceptualize it for a few reasons: 1) their illness course is often shorter than medical patients making the cumulative time to acquire fluids less 2) overall (not all) surgical patients (eg trauma) have less cardiomyopathy than medical icu patients (a bit of a generalization) 3) real fluid losses exist! Drains, third spacing etc. The hardest point for me is figuring out when this phase should be considered done. Treating them statically like this for too long can lead to problems for complicated patients requiring prolonged hospital / icu stays. Interested in some other surgical intensivist thoughts @ewwalser @rob_leeper @traumaicurounds

(4/x) Even if a patient is fluid responsive (they will increase their stroke volume with a fluid bolus), this doesn't mean that the best way to improve their stroke volume is fluids. For example, in a patient with low mean systemic filling pressures from sepsis/vasoplegia, vasopressors can increase venous tone, venous return, and stroke volume. A collapsed IVC does not need to be filled with fluids!
