@fionnamartin @GSTTGeriatrics and at some point you realise either that you no longer have the energy/health to keep the daily grind, or that things have changed so much it is not your era any more and you are out of your time Sometimes ego & fear of retirement& loss of identity/purpose keep people going

How many more ? @doctor_katie @timfarron @drlukeevans The list of surgeries who took issue with the NHS across the North of England back in 2015 is dwindling. They’ve almost all now gone - the end of the family doctor service.
Andy Pow@medicaccountant

A sad day for rural small general practice. Reeth was part of the small surgeries group in 2015 which highlighted what would eventually happen with cuts in funding. They become unsustainable when the incumbents go. Losing a surgery is terrible. The NHS were warned. It failed.

On a day when the HSJ published a story about the NHS CEO planning for a non doctor service - particularly in coastal & rural areas - Reeth announced its closure. This sign will become a relic of rural life.

@GlasgowRugby_ TBF Bath were on a team warning. Ref was crystal clear if another penalty a player would be in the bin. As long as he is consistent I don’t think we can grumble and I’m a Bath fan!

That kind of decision shows that refs still have too much control over the outcome of games. A yellow card for being marginally offside is mental #SALvBAT

@naomi2009 @rayaddisonlive They were effective at giving patients quick access to primary care services but they were vastly more expensive than delivering GP services so they were scrapped. If the equivalent funding were put into GP practices today we would be in a very different place. @DrSteveTaylor

@rayaddisonlive This is where urgent walk in centres should come in - places you can see a GP without an appointment - walk-in service - but not for broken arms, cuts and so on. What has happened to these?

Hellish day in A&E. The doctors and nurses are working flat out but the constant stream of patients is just too much. When I was a kid if you went to A&E you saw people with broken arms and bloody eye patches, things cut off or sticking out of their head. Today it was just people who couldn't get a GP appointment. Why do we not have enough doctor's surgeries in this country? Is it financially unviable to run one, too expensive to become a GP, or no longer a sought after career? What the hell is going on?

Maro Itoje shouldn’t have shoved player and argued with ref at end as could have conceded 10 metres. the ridiculous spectacle of Pollock making fun of crowd and shoving opponents is fatuous and could be poisonous though france will appreciate his late knock on

Why on earth didn’t Chessum go in nearer the posts to make the conversion easier? That’s 2 points just given away. #FRAvENG

All economic analyses show how much more cost effective are GPs than hospitals. For primary and secondary prevention, diabetes, hypertension, asthma, elderly medicine, mental health, child health, and addictions.
Dr Steve Taylor@DrSteveTaylor

The ‘murky’ world of GP finance This is an example of a 10,000 Patient practice The average patient will have 7 GP practice appts/year, 1 A&E attendance every 3 years, 2 hospital appts/year £169/patient/year goes GP £3079 goes to hospitals £40pa would solve GP crisis

@DanNeidle @paullewismoney Same effect as the £100k threshold trap - many professionals reduce/limit hours so that they don’t cross over into very high marginal rates. Any tax system that disincentivises working needs to be scrapped/ amended ASAP.

@paullewismoney Paul, it’s not the admin - it’s the 20% hit to profits and/or increase in prices. Absolutely rational for micro businesses to constrain their growth. Absolutely awful macro consequences.

If you ever wondered what cutting your nose off to spite your face means, it is this bit.ly/4rFjPxa cutting the turnover of your small business to avoid registering for VAT, which anyone with a spreadsheet can sort out in half a day every three months
