@DrSarahAxelrath One potential issue is when the training is wrong, e.g. medical training standard procedure for prescribing antibiotics for a UTI may be flawed. Yet some MDs will hold to the training even if doesn't match reality x.com/AshleyGWinter/…
Ashley Winter MD || Urologist@AshleyGWinter

The most common lab criteria for UTI, >100,000 cFu/ml, was derived from one study over half a century ago in asymptomatic patients and makes no sense for defining a UTI in SYMPTOMATIC patients.

I already regret saying this, but: doctors train for 7+ years to learn how to gate-keep thousands of medications appropriately, balancing the rights of patients to the most effective treatments with the inherent risks of every medication, which also vary by patient.

People seem to be really invested in the belief that doctors are withholding the perfect medications from them, for no reason. Question to those people: what do you think would happen if all medications were readily available to everyone OTC without a doctors’ consultation?

My soul dog died this week. He was the dog I wanted my entire life, until one day I realized I was a home-owning adult and could totally get a dog my own damn self. I would have gone to hell and back for my dog. Goodbye, Fawkes. 💔 I made a memorial ring with a bit of fur.

@metaphorician Since you already seem to be going near there, might want to consider the hike over the High Tatras, beautiful place and nice mountain huts and lodges.

@angelasjaded Anger is natural and the inclination to stop it is a wholesome one. While we should never feel ashamed of feeling it, even righteous anger contributes to overall pool of anger in the world. Thus, when detected, we should redirect our energy into expanding the world supply of love

@PhysicianDoodle Wow. This would be a good reference image for anytime a MD dismisses a female patient's uterine issues / pain.

I used the free version of ChatGPT to discuss my ongoing Crohn's disease treatment, to see what it would be like for someone who doesn't have access to plus or pro but was struggling with the same issue as me. The answers were so compelling and reassuring that I got slightly emotional. This is the most important thing anyone has ever made. We have created truly scaled compassion, backed by a mind smart enough to know what it's talking about. Free access to this is one of the greatest humanitarian projects ever conceived.

@BocaDuk3 @JudiciaryGOP 20.5k is only MDs, are also ~8k DOs (functionally equivalent). 5k U.S. citizens went to international medical school. So actually have ~35k-ish U.S. citizens. 95%-ish of U.S. citizen from U.S. med schools successfully match vs. 65%-ish for U.S. citizens from int'l med schools

@JudiciaryGOP Silly report. US medical schools graduate only 20,500 people and there’s 43,500 residency openings. Not everyone is entitled to their dream specialty. Life is hard, maybe you weren’t cut to be a doctor.

#BREAKING: New Report Exposes How Medical Residency Hiring Monopoly Harms Patients and Doctors Newly obtained documents reveal how the Match placement system for resident physicians operates as a monopoly in the medical residency hiring market. Its monopolistic practices harm resident physicians, impede patients' access to care, and constrain the growth of America's physician workforce. A special-interest antitrust exemption currently shields the Match’s anticompetitive conduct from scrutiny, allowing it to harm the public while avoiding judicial oversight. Read the full report here: judiciary.house.gov/sites/evo-subs…

@gptbrooke @kramerposts It's a tough problem. Various ic.org courses talk about it. I've taken a few of them thinking there is perhaps some great answer, but in my opinion banning some difficult people from communities (and dealing with the flack received from doing so) is a necessity.

@kramerposts it even happens with vibecamp though! it's a small small number of unhappy people who a disproportionate drain on our resources and that just fractals out through society, up to the highest levels

Current status: feeling frustrated that the whole “the bulk of shoplifting, car thefts etc are committed by a small group of very active criminals” thing applies to EVERYTHING it’s a small handful of people causing things to be worse for everyone, no matter what rock you look under I wonder if we’ll ever be able to fix this, it’s gotta be a brain thing right? Will we(/should we even if we can?) be able to idk increase empathy or impulse control with targeted ultrasound one day

“Ivan Illich, wrote in "Deschooling Society" (1971) that schools don't just teach, they teach people to need to be taught..They systematically replace the learner's internal sense of competence with external certification. The institution becomes the arbiter of when you're ready”

@sardonicsqueak More common amongst teenagers I imagine. There's a great research paper / book about the unconcious patterns of interaction people can fall into called "Games People Play", short enjoyable and interesting read.

Please keep me in your thoughts and prayers over the next few weeks. They will determine the trajectory of how things will go for the next while. I’d appreciate support and DMs, as well. The bright side: winter is leaving, I have a job, and there is still hope for the future
