From talented EP fellow to rising junior faculty star, @ChauVoMD has made the #AblateVT team better @MUSC_EP w/ her commitment to patient care and comprehension of substrates and mechanism -- as evidenced by another weekend VT case for a storming patient done today: Note ILAM w/ clustering of low voltage and DZ in area of lateral LV adjacent to paced LV; similarly note clinical VT activation on LV lead with LAT ~0ms relative to FF egm. Maps of two VTs, including the area correlating w/ DZ and adjacent LOBs, shown below thanks to @AbbottCardio mapper @AMatthews0. Great work by all care team members who selflessly came in for device cases on Saturday followed by this VT on a Sunday. What are y’all’s approaches to mitral flutter with PFA? Redo case presenting in mitral AFL. Placed anterior line due to presence of some scar. Basket config near valve then flower once further up once able to get catheter to lay flat against tissue. @BSCCardiology #farapulse #EPeeps

@DrRoderickTung Interesting development. Thanks for sharing. I’m still banking on LV septal pacing being a back up whenever selective capture not achievable with a few studies suggesting may be as good as BiV pacing. Larger studies and better tools needed!

Going to say it again… 1998 conversion factor for doctors - $36.68 2024 conversation factor for doctors - $33.29 1999 Insurance premiums - $6k 2024 Insurance premiums - $24k When you pay more for healthcare, it’s not coming from private, independent doctors no matter what the insurers, politicians, or news stories say. Physicians are paid less in absolute terms over the last 25 years while insurers are up 314% and inflation is up 83% We need a long term solution to ensure physicians are paid commensurate with their cost of doing business. Our country is on an unsustainable path in healthcare where all of the profits are going to insurers and hospitals, leaving physicians no choice but to vertically integrate, which drives costs even higher.

@mbelshazly Depends on the quality of previous ablations I would also add! …but agree. At a certain point, if the whole left atrium is ablated and not contractile there is marginal or no benefit.
