@NeilFlochMD @spenceralex @TaraBull808 While I agree with you on this point, a large portion of the surgical hernia community would argue that diastasis can be painful and debilitating and needs to be repaired thru MIS hernia/plication techniques or open plication. I decide on a case by case basis.

A patient is critically ill with peritonitis and free air; you have reason to believe this is probably a perforated ulcer. There is a long midline incision from xiphoid to below the umbilicus. Family says this was a ventral hernia with mesh. What is your surgical approach?

@Dr_AhmedAdil @DissanaikeMD @rbarbosa91 I have seen injury from all three techniques. Use what you are comfortable with and with repetition you will be very successful understanding that with any entry technique there is always a potential risk of an injury.

@DissanaikeMD Agree. Hasson open method trocar entry is much safer than the Verres needle insertion. But do you think that the abdominal wall is hard like the cardboard box?! @rbarbosa91
