Video-based assessment tool for workflow analysis in minimally invasive colorectal surgery: expert consensus-based development and multicentre validation of ColoWorkflow ➡️doi.org/10.1093/bjsope… ColoWorkflow, the first video-based assessment tool for workflow analysis across minimally invasive colorectal procedures, was based on a modified Delphi process involving over 40 experts from 11 countries. Validation on a multicentre data set demonstrated moderate-to-strong inter-rater reliability and broad applicability across five major colorectal procedures. Quality metrics for perioperative red blood cell transfusion in gastrointestinal cancer surgery: international Delphi consensus ➡️doi.org/10.1093/bjsope… A consensus on quality metrics to assess and monitor the use of perioperative red blood cell transfusion (RBCT) in gastrointestinal cancer surgery was generated through a four-round modified Delphi process with 37 international experts. Consensus was reached on 18 quality metrics across structures, processes, and outcomes of care, reflecting both clinical importance and feasibility. Clinical Outcomes Associated with Prior GLP-1 Exposure in Burned Patients ➡️doi.org/10.1093/bjs/zn… 💉 GLP-1 receptor agonists may offer benefits beyond glycaemic control in burn patients, with potential anti-inflammatory and immunomodulatory effects. 📊 In a large matched cohort (n=8,307 per group), prior GLP-1 use was associated with 54% lower mortality (OR 0.46; p<0.001). 🏥 Significant reductions were seen in ICU admission (−35%) and intubation (−62%), indicating lower critical care needs. 🦠 Infectious complications were also reduced, including sepsis (−32%), pneumonia (−24%), and MRSA (−27%), with less antibiotic use. ⚠️ A signal toward increased hypertrophic scarring (+27%) was observed at longer follow-up. 🎯 Prior GLP-1 exposure may improve burn outcomes, supporting further prospective studies to guide perioperative strategies. Differences in recovery time between trapeziectomy and carpometacarpal joint replacement: meta-analysis ➡️ doi.org/10.1093/bjsope… This study aimed to define and compare patient-reported recovery trajectories following trapeziectomy and carpometacarpal (CMC) joint replacement for thumb-base osteoarthritis. This study provides level II evidence that CMC joint replacement results in faster recovery than trapeziectomy over the first 3 months after surgery, with newer dual-mobility implants associated with quicker recovery in DASH and key pinch strength than single-mobility implants. Sleeve gastrectomy versus Roux-en-Y-gastric bypass in patients with body mass index over 50 kg/m2: international multicentre cohort ➡️doi.org/10.1093/bjsope… This retrospective multicentre cohort study included 3978 patients after sleeve gastrectomy and Roux-en-Y-gastric bypass (RYGB) with initial body mass index > 50 kg/m2. Scheduled intravenous acetaminophen versus thoracic epidural analgesia for postoperative pain control after minimally invasive gastrectomy: multicentre randomized non-inferiority trial ➡️doi.org/10.1093/bjsope… Thoracic epidural analgesia (TEA) is commonly used after abdominal surgery, but its necessity in minimally invasive gastrectomy remains uncertain. In this pragmatic multicentre randomized non-inferiority trial, scheduled intravenous acetaminophen combined with wound infiltration achieved non-inferior postoperative pain control compared with TEA, with comparable pain trajectories and recovery, and no increase in adverse events. Postpancreatectomy diarrhoea: prospective, single-centre longitudinal analysis of incidence, risk factors, management, and impact on quality of life ➡️doi.org/10.1093/bjsope… Postpancreatectomy diarrhoea affects over one-third of patients despite appropriate enzyme replacement, significantly impairing quality of life. This prospective study identified key risk factors for moderate-to-severe diarrhoea, including vascular resection, arterial divestment, and pancreatic ductal adenocarcinoma aetiology. Day case and inpatient elective thyroid lobectomy across England: observational study of variation in practice and safety ➡️ doi.org/10.1093/bjs/zn… 🏥 Day-case thyroid surgery remains underused, with only 9.9% of 41,518 procedures performed as day cases across England and wide variation between centres (0–74.6% 🔍 Day-case surgery was associated with a lower 30-day readmission rate (OR 0.73; P=0.021), with no signal of worse outcomes ⚠️ There was no increase in complications, including haemorrhage, in day-case patients 📊 Centres with higher day-case rates (>30%) did not demonstrate worse outcomes compared with low-use centres 🎯 In appropriately selected patients, day-case thyroid lobectomy appears safe and should be considered for wider adoption.
