Our build path for biologics is Proteus as the intelligence layer, then a closed loop that feeds into it and pushes toward a preclinical candidate. The Moderna/Merck individualised cancer vaccine work is a useful precedent, having a bespoke payload and a standardised process. Compassionate use may matter in edge cases, but the bigger point is making patient-specific biologic discovery fast and repeatable enough to produce real preclinical candidates, not just interesting sequences.

Most of “personalised medicine” is still personalised diagnosis. Genome. Blood. Wearables. Imaging. Screens. then what? If all that ends in “you have a rare mutation, here is the closest generic treatment,” that's just personalised bad news.

5. Extremely personalized cancer medicine is hard. One of the hard parts is that you can't really have cheap AND fast and you /need/ fast in these cases. It's also really hard to compare outcomes because each treatment is different. This is legitimately hard!

Culture was possibly the key breakthrough in human history. Once we could share ideas, learning became much more efficient, and complex technologies became possible. LLMs haven't yet built their own culture or organisations. But it seems plausible they will. On the podcast last year, @karpathy speculated about what it will take for LLMs to build their own culture, and why they're not there yet.
