This analysis showed -almost all patients with ACS had detectable plaque at their prior ccta -the plaque burden before acs is high -patients <50 years had lower plaque burden but lots of high risk plaque - proportion of calcium in plaque increases much with older age

Why I hate CT calcium scores: 70 year old woman insisted on getting one, so I ordered, she's in the 85%-tile. She's a treated diabetic, treated with statins for cholesterol. No chest pain. SPECT imaging cardiac study and echo are unremarkable. The CT gave me no additional info.

For our trainees and early career docs, how can we resolve the opposing views on role of CAC from @RFRedberg, @DavidLBrownMD, and @drjohnm with those of @khurramn1, @BudoffMd, and @MichaelJBlaha ? Is there time, $, interest to power an outcomes-based RCT for a diagnostic test?

CAC scans offer no clinical benefits, often lead to more tests and worry, incidental findings and radiation risk. Need to rethink their use. Time for an RCT!
