The quoted image doesn’t really look like an abdomen that was sufficiently gravid to warrant a caesarean section. My more likely impression is that this may have been an exploratory laparotomy, possibly for an ectopic pregnancy. The abdominal wall does not show the typical features of prior significant distension from pregnancy. One key point is the challenge of achieving a neat subcuticular closure like this in a recently gravid abdomen. The skin in such cases is often stretched and lax, which can make precise approximation more difficult and may affect the cosmetic outcome. The picture below is one of my subcuticular suturing for an immediate Post Gravid abdomen after I did a CS. See the difference.
