Pt with SOB. CT notes ascending aortic thrombus. Despite anticoag, has CVA w/in 24h of initial CT. Residual clot in TEE “blind spot” ->ICE guided aspiration with angiovac. ICE introduced via left brachial artery. Just seen for his yearly follow up, doing great! Done with @SEH_MD

Case of the week!! woman in her 60s, history of MI, comes with acute non-disabling stroke. Found to have large, mobile massess in the LV. Underwent transcatheter Angiovac left ventricular thrombectomy with bilateral Sentinel embolic protection. Attempted antegrade transeptal approach but could not advance C180 cannula through the mitral valve. Switched to retrograde transaortic with F18 cannula with successful debulking and retrieval. Reperfusion ECMO cannula switched from fem artery to fem vein.
