The evolution of thoracoabdominal aortic aneurysm repair from 1950s to 2024 is nothing short of unbelievable. Back then it would have been fantasy to think that a patient could return home postop day 2 after treatment for extent 2 TAAA with chronic dissection.

@farkomd Always a 2 am call, often misdiagnosed and were already playing catch up. Are you approaching them mostly endo or open?

@drseanlyden 💯 agree with you on this! We have very little in common anymore with our General Surgery colleagues. We definitely don't need their governance of our specialty and how we certify and practice.

What is happening with the SVS Task Force evaluating the concept of an independent board? Who is on it? Why are they not educating and polling our membership of the pros and cons? Cardiovascular medicine is now applying and has full societal support. What is the downside?

@westleyohman @farkomd That's how I used to do it, too! Going from the subclavian to the carotid allows you to parachute the LSCA anastomosis and see the heel and the toe well, allows transection and oversewing the stump of LCC without the graft in the way. Try it next time and I bet you switch!

@AorticCenter @farkomd We’re completely out of order! I do the R CCA, then L SCA, then transpose the L CCA onto the graft after it’s tensioned appropriately.
