Very excited to welcome our incoming Emory Pathology residents and celebrating the Match Day with desserts and special match cookies made by one of residents 😋🥳
I still remember the excitement of opening that Match email back in March 2017, and feeling incredibly fortunate to have matched at my top choice. Now, nine years later, it’s just as special to experience that excitement from the residency leadership side.
So much effort goes into this process from both applicants and programs. Congratulations to everyone who matched this year, and a warm welcome to our newest Emory trainees. We’re thrilled to have you join us and look forward to supporting you on this journey!

🧵 Case highlight:
Female patient with acute kidney injury and history of breast cancer, treated with chemotherapy.
Renal biopsy (PAS) shows atypical hyperchromatic cells occluding glomerular intracapillary lumina. PAS highlights segmental double contouring of capillary loops, suggesting endothelial injury.
Immunohistochemistry confirms the origin:
🔹 Tumor cells positive for TRPS-1 and GATA3 → consistent with metastatic breast carcinoma
🔹 Enlarged endothelial cells highlighted by ERG immunostain
Diagnosis: Glomerular metastatic carcinoma with intracapillary tumor emboli and secondary endothelial injury.
In patients with prior malignancy and chemotherapy exposure, unexplained AKI should prompt consideration of intraglomerular tumor involvement.

Fat necrosis is a great pretender. 🫧 It mimics malignancy clinically and radiologically. Reactive fibrosis (black circle) creates a spiculated edge.

This is an inflamed blood vessel in a background of pneumocystis pneumonia ("froth with dots"). The teaching point is: mycobacterial and fungal infections often feature a striking vascular infiltrate rich in lymphocytes. This is not a true autoimmune type vasculitis.
