The first of two teachings for the Discharge Coordinators - learning how to use the handover and referral function on CareFlow Connect, including referrals to identify patients for R2G and the Homeless Persons Coordinator. 👏👏 @digitalbhrut @BHRUT_NHS

Super proud of @MikeExford and @jojo_mitchell_x finally getting their certificates for @BHR_thePRIDEway for leaders. They completed this during the pandemic and their ability to use the tools learnt reflects in their work every day @BHRUT_NHS

Harm caused by hospital admission

Recovery plans are in progress within the red2green team over the coming weeks we will be developing and launching our new programme to help reduce delays and improve our 4 key metrics - pre 12 and pre 5 discharges, reducing our 7 and 21 day length of stay @BHRUT_NHS

Over the last 3 months we have been working to increase our discharge coordinators to support our wards to improve early discharges and reduce delays related to discharge, I am happy to announce we now have a full team and look forward to developing this role @BHRUT_NHS
