There are only 3 options here. a) Trump is so stupid that he thinks that the image really did depict him as a doctor. b) Trump is completely senile now, and we should 25th Amendment him. c) Trump committed Blasphemy.

Trump needs to be removed from office; he has clearly lost what little control of himself he had. He is a disgrace. The only good thing is that his derangement is becoming clearer even to his strongest supporters, so maybe the country can salvage its reputation after he's gone.
