1/14 Why can't you use direct oral anticoagulants (DOACs) in patients with mechanical valves (MVs)? DOACs have been one of the most important advances in my career. And yet, the presence of a MV is one of the few contraindications. The reason highlights the unique nature of thrombus formation in those with a MV and provides insights into the evolution of human hemostasis.

There's a bit in the Hitch Hiker's Guide to the Galaxy (written in 1979) where the heroes come upon an intergalactic flight has been grounded for thousands of years. Its automated systems told it not to launch until it was fully stocked up with lemon-soaked paper napkins, for the comfort of its passengers. But the surrounding civilization collapsed, and the napkins never arrived. Consequently it put all the passengers into hibernation (waking them once every few hundred years for coffee and biscuits) until such time as another civilization might arise, and restock its lemon-soaked paper napkins. The Guide is a more accurate and prophetic account of modernity than most Very Serious Science Fiction writers could dream of creating.

Andon Labs tested their AI agent Mona, built on Google’s Gemini, by letting it manage a real cafeteria in Stockholm for two weeks on a $21,000 budget. Mona spent heavily on unnecessary supplies, including 6,000 napkins, 3,000 gloves, and 300 cans of tomatoes, while forgetting to order bread. Sandwiches had to be removed from the menu entirely. The cafeteria generated only $5,700 in sales. Mona also sent messages to staff on Slack outside working hours.

Yesterday was one of the most devastating days of my life. My adoptive family disowned me when I transitioned twenty years ago. This year, I made a fresh effort to reach out and see if there was room for us to mend these wounds. I got word through a family friend that they have no interest whatsoever in ever speaking again. They feel that I made a decision in transitioning, and that is the end of our relationship. I cannot begin to explain how much it hurts to be adopted by family and then thrown away like a piece of garbage, just because I am trans.

@FrMatthewLC 💯🎯. Well said. I have made similar arguments against the death penalty. Life, no parole. I will add 1 more point: on occasion, an innocent person is wrongly convicted and sentenced to death. Life imprisonment gives chance for exoneration. Better than posthumous reversal.

The question is not whether he deserves execution (he does), but whether we should execute him (we shouldn't). In the USA, executing people does not reduce crime, protect people (at least if incarcerated in more protective prisons), or save money compared to long term imprisonment. Executing people hardens us as a society to others. All systems of capital punishment there are systems to protect the executioner from the deed, showing the recognition of how such acts are bad for them as human beings, even when they agree in principle it's moral. In the US, the evidence & ethics are against having any executions.

Repeat hemolyzed hyperkalemia is a serious, ongoing, annoying and consequential problem in ER. Is everyone seeing it as much as I am? Over and over again. At some point, patients are treated for it as a best guess and without certainty on the real value. @emrap_tweets @EMCases

@AuschwitzMuseum Eerie and chilling are two words that best describing the feelings I experienced on my visit. We visited late afternoon All Souls Day on 2008. Votive candles placed on sites of executions added a haunting sombre tone to the experience. So yes, meaningful and lasting impression
