Yesterday, I did the first BASILICA in my new place after starting an electrosurgery program over the past few months. A few things I learned so far:
• Didn’t have an Astato 300 cm, only 180 cm, workable, but takes a few more steps.
• Confianza Pro 12 wasn’t as effective as Astato 20 for traversal. I’d use the short Astato to cross, then cross the leaflet with the microcatheter and exchange for a 300 cm Confianza for the flying V
• Corsair Pro can still get the job done without piggyback, a 0.035 Torquer is needed
• Smaller gooseneck snares can work if you don’t have the ideal size

When doing 3D, the upper left 2D image is called “2D reference plane”. Some people call it “the starting 2D image” which is less appropriate IMHO. Hope the 3D echo experts would agree @lpbadano
