First retrograde RCA CTO PCI at my new hospital using S-base and reverse CART. - Bifemoral, 8 F EBU 4 90cm, 7F AL1 100 cm. - Retrograde euipment (mamba flex 150 cm, sion black, suoh, gladius, gladius mongo) - Antegrade equipment (turnpike spiral 135 cm, 7F guideliner, minamo, gladium mongo, 2.5 and 3.0 balloons for SBASE and Reverse CART) Great learning case and a good reminder of some CTO basics: • Collaterals can be deceptive, not all communicate with the PDA, and they may overlap in very tricky ways. Orthogonal views are essential before advancing the microcatheter. • Fail fast and move on. If a septal branch isn’t working, switch early. If Sion Black isn’t effective, SUOH 3 is a good alternative for septal surfing. • A TrapLiner can make things smoother. I didnt have it. It is manageable, but it may add a few extra steps. • S-base is helpful when there’s a side branch at an ambiguous proximal cap. Use that to your advantage. • After externalization with R350, keeping the microcatheter in the distal PDA helps reduce the risk of septal collateral injury. • Before removing the retrograde wire, a final angiographic shot is important to rule out perfs, especially if dual embolization is needed. We are launching a U.S. multicenter registry studying the UNICORN technique during valve-in-valve TAVR. We are inviting centers with UNICORN cases to collaborate and contribute retrospective cases. Collaborators will be included in resulting abstracts/manuscripts.
