When the system is under pressure, the focus is ever more on the hospital. We are not meeting people's needs in the community, especially for chronic disease management and care. Unless we address this (and GPs play a key part) we will never improve the situation we are in.

My direct, heartfelt appeal in today’s @guardian to @wesstreeting & @Keir_Starmer in the light of yesterday's vote on AD. If you sincerely care about dying people, surely you will now publicly & urgently commit to proper funding for our threadbare palliative care?

Whstever your view on #AssistedDying , it is vital that #PalliativeCare services are improved in the UK, regardless of whether the law changes. Services to support people with life limiting conditions need to be properly funded and not reliant on goodwill and charity.

Interesting afternoon at @RCGPWales Council meeting. Good discussions about state of General Practice and what we can do to support the profession. Also some thoughtful debates about issues going to UK council. An engaged bunch of people, and well chaired by @ChristmasRowena
