After Kiley was diagnosed with glioblastoma, an aggressive form of brain cancer, doctors told her she might have only months to live. At 18, she wrote a will, chose a casket, and began living scan to scan. But, over the next five years, she kept on living, defying expectations. Her scans stayed stable. Why? How? After a recent MRI, her husband @ColeLemasters used ChatGPT to help make sense of the dense language in her radiology report. That conversation led him back to Kiley’s original biopsy records, where ChatGPT helped surface a crucial detail: an IDH1 mutation. In partnership with her neuro-oncologist, he then confirmed what that meant. Under updated medical standards, Kiley’s tumor was not glioblastoma, but an IDH-mutant astrocytoma: still serious, but with a very different prognosis. Instead of bracing for the worst, Kiley and her doctors began focusing her treatment plan on long-term care. Now, after years of uncertainty, she and Cole are planning for a future.

I think it's pretty obvious how this went down, just as you said: they did her IDH1 status at the very beginning, because this was also standard back in 2020. Then this went into her file and when ChatGPT browsed her file with the knowledge of 2023/2024, it flagged the inconsistency between the diagnosis of glioblastoma and IDH1 mutant. Yes, they should have communicated the change of guidelines with her. My guess is that the IDH1 mutation went missing and she was just considered an exceptionally great course of a glioblastoma.

This is something I can speak to personally. @ColeLemasters let’s connect. My wife has been living with astrocytoma for almost eight years now. There may be more to the story than the post shows, but IDH mutation status is one of the first things any major brain tumor center should test for. The fact that you found it rather than her care team is alarming. My guess is that she was classified as GBM under the 2016 WHO guidelines. The 2021 update reclassified these tumors based on molecular markers, so an IDH-mutant tumor called glioblastoma in 2016 would now be an IDH-mutant astrocytoma, grade 3 or 4. WHO essentially pulled IDH-mutant tumors out of the glioblastoma category because those patients tend to do better.

Don’t mind me as I faithfully watch the Royals continually dominate the month of February just to burn out in May, for they are 3-1 so far this young spring, so it appears the cycle shall continue. In a weird, paradoxical, way, hopefully the Royals suck over the next few weeks.
