@_JuliaRx No consensus for low. Intensive induction use posacazole. For HMA/Ven could do none, posaconazole, or fluconazole? I consider geographical location, time of year, and pre-diagnosis cytopenias to individualize. 50-100 Ven target, no need for ramp for most.

Polling #oncopharm what anti-fungal prophylaxis strategy does your institution utilize in AML induction (intensive and non-intensive)? Do you avoid azoles or make dose reductions on days venetoclax is given? Would love to hear what other centers are doing!
