Streeting says he will soon roll out patient power payments, where patients will decide if the provider should get the full cost of their care based on the quality of care. Starting with women he says this will "kick medical misogyny where it it hurts."

You can be a doctor who’s been working for 5 years, hold MRCP and almost be a registrar, but your Mother will still give you advice about minor medical maladies I assume this will continue if I ever become a consultant

Maybe I’m becoming desensitised but this hasn’t hit me as much as the photos of malnourished babies in Gaza did.

Most universal healthcare systems train competent doctors in 5–6 years. Only the NHS stretches “training” to 8–11 years by using juniors to plug rota gaps and cover service work that has nothing to do with learning. That’s not higher standards — it’s a structural choice that’s now breaking the pipeline.

@juliaisobela is right! 50mg sertraline + 70mg Vyvanse (Lisdexamphetamine) daily here. How can I ask my patients to overcome their shame if I hold my own? I can't Fortunately, I have no shame. What do you do? My best.

Today I went to clozapine clinic for my monthly bloods + picking up my meds Clozapine has been instrumental in keeping me mentally well & I’m not ashamed to have to take daily meds for my mental health, even as a doctor myself *nobody* should be ashamed for taking MH meds

@AriReddy Amen.. I'm in Australia now as an SHO studied in Edinburgh. FY in Glasgow. After 3 years I sometimes feel I have the same skills as an Australian intern... And I'm now almost 5 years post exams so forgetting the knowledge after years of service not using it. #byebyenhs

1️⃣ Most people don’t realize how unusual the UK’s doctor-training model is until they compare it internationally. France, Germany, Australia, the Netherlands — all move doctors to independent practice years faster. The UK isn’t the norm. It’s the outlier.

@EISSEM @EMManchester Every cubicle here has a computer plus the dreaded COW. If that’s not the case then dreadfully inefficient.

@EMManchester It’s great for chasing up results and requesting further investigations and making referrals. Can do it all from the one spot. I type as I take the Hx. Not ideal but tried the scribble notes then type after option and it was too inefficient.

@kcisc I wasn't doing that independently as an FY2 ... Dodgy. AF. Also...why would a physio WANT to sutures and assess head injuries? That's...not their job. Like why would a hairdresser wanna be a manicurist just cause they notice nails?

I took the screenshot on the left from an official NHS website pushing the 'advanced' roles. A physio (who has many people he disagrees with blocked - hence the quoting) who is staunchly defending expanding roles of physios in primary care, agrees it is out of their scope,

Got my bloods done today...then called into work for overtime as understaffed. It's no fun being on both sides. HR 100-130 at rest with palpitations & pressure often for a few hrs at a time . Going on for months. FINALLY got my ass to phleb for baseline bloods. Wtf, body?

@Oxfordite Interesting! I have started taking a multivitamin as my diet is awful wondering if I had any deficiencies but worth checking! I mean I am anxious but it's NHS trauma, I think I heal ok.. I don't damage myself much so not sure haha.

@EISSEM Not advice.. but memorable person with such a racing 'baseline'(!) HR was vitamin A deficient.. UK ~only tests gastric surgery patients - maybe Australia is more flexible? Review your dietary sources and symptom list (night vision, bright light, skin, anxiety, slow to heal..) 🙏
