Most people assume that if the NHS needs more specialist doctors, the Government would train more of them. But that’s not actually what’s happening. The Government can decide how many specialist training jobs exist each year. They can increase them, reduce them, or remove them entirely. These numbers are a political decision. So when the Government removes 1,000 future NHS specialist training jobs, that is an active choice to have 1,000 fewer future NHS specialists. That means fewer potential radiologists reading scans. Fewer potential surgeons doing operations. Fewer potential anaesthetists running theatres. Fewer potential psychiatrists and GPs seeing patients. At a time when waiting lists are in the millions and patients are waiting months or years to see specialists, the Government has actively chosen to reduce the number of future specialists. That doesn’t just punish doctors. It punishes patients and the NHS as a whole, because it means fewer potential specialists and longer waits in the future. And the most concerning part is why this happened. These training jobs were discussed in the context of negotiations with doctors. That means specialist training jobs, and therefore future NHS specialists, were being treated as something that could be added or removed depending on whether doctors accepted Government terms. That is not workforce planning. That is using future NHS specialists as leverage. The Government can create more NHS specialists if it wants to. It can reduce waiting lists faster if it wants to. This isn’t even capitalism. It’s medieval lordship. Father starts a war. Sons sell the weapons to countries that need his protection to survive. Buyers know the purchase is tribute. Saddam Hussein would have recognised it instantly. The US isn’t a democracy right now. It’s a protection racket with a flag. Britain needs to face that fact - and build something different with Europe before it’s too late.

It is almost unbelievable that the government is on the brink of allowing the water companies to scam the British people *once again* by letting them get away with serious criminal offences without any actual plan & commitments for fixing illegal outfalls. I will use one of the most polluting outfalls on the Roding to illustrate why this is a terrible idea. The aptly named ‘Cascades’ combined sewer overflow (CSO) discharges almost every time it rains (even lightly) for more than a few hours. It discharges more than 400 hours per year, the majority of which hours of spilling are completely illegal. Worst of all, my testing of the outfall over the past year has shown that the volume (over 80 litres a second) and concentration (up to 50ppm ammonia) means this CSO is highly damaging, effectively turning the whole river into a sewer for over half a mile downstream. What will be the effect of letting Thames Water off fines for this illegal outfall? 🧵
