Reflections on NYC CTO-PCI 2026: The Shift Toward PJWs and Knuckling #CTO #CTO2026 IMHO 1. The Transition in Wire Selection The initial wire choice in CTO-PCI has undergone a significant transformation. Traditional active wiring using high-torque wires like the Gaia series is increasingly rare. Instead, Plastic Jacketed Wires (PJWs) have become the primary tools for the initial approach. 2. The Rationality of the Knuckle Wire Technique (KWT) The routine use of knuckling with PJWs is highly rational. This approach markedly reduces the risk of perforation, making it a safer and more efficient strategy for subintimal tracking compared to previous methods. The synergy between PJWs and knuckling is clearly a superior safeguard against procedural complications. 3. Proactive Information Gathering (#HDR & #Carlino) Moving forward, we should more aggressively utilize HDR (High-Definition Radiography) and the Carlino technique(controlled contrast injection) to maximize the acquisition of visual information. It is evident that the current trend toward PJW usage is naturally evolving into an HDR-driven strategy. By intentionally utilizing these modalities, we can gain a clearer anatomical understanding of the occluded segment. 4. Entering a New Era of CTO Intervention We have entered a new era of CTO-PCI that demands sophisticated mastery over a diverse technical toolkit: Precise selection and differentiation of PJWs. Expertise in KWT (Knuckle Wire Technique) and STAR (Subintimal Tracking and Re-entry). The strategic integration of HDR and Carlino. Key Takeaways Safety First: PJWs + Knuckling = Significant reduction in perforation risk. From "Blind" to "Visual": Proactive use of HDR and Carlino is essential for anatomical clarification. Evolution: The current dominance of PJWs is a precursor to the widespread adoption of HDR-guided procedures.
