Clinical deterioration is an underappreciated risk of ED boarding.
In a multi-center study of >173,000 admissions, nearly 1 in 25 ED boarders admitted to the floor experienced early clinical deterioration requiring an admission to an advanced level of care, with almost half occurring before leaving the ED. Each additional hour of boarding was independently associated with an increased risk of deterioration, and deterioration carried substantially higher 28-day mortality.
Identifying high-risk patients early may prevent avoidable escalations in care and patient harm.

🔍 What the study looked at
A retrospective cohort study across 3 EDs in England (2023–2025) examining:
• 49,000+ medical admissions
• 210,000+ non-admitted ED patients
• 88,000+ ambulance handovers
• 146,000+ ambulance responses
The study explored both direct patient harm and system-wide effects of boarding.
📊 Key findings
⚠️ Boarding increases patient harm
• Each extra 4 hours of boarding →
→ +8.6 hours inpatient length of stay
→ +8.4% higher odds of 30-day mortality
→ increased re-admissions
🚑 System-wide consequences
• More boarding → slower ambulance response times
• Increased ambulance handover delays
• Longer ED stays for other patients
• Reduced bed availability across the system
🏥 Medical patients accounted for 81% of total ED boarding time.
💡 Why this matters
Boarding is not just a flow problem — it is associated with measurable harm to patients and reduced efficiency across urgent and emergency care.
Reducing boarding could:
✅ improve patient outcomes
✅ improve ambulance response times
✅ increase hospital capacity
✅ reduce system pressure
This paper adds important evidence to ongoing conversations about ED crowding, patient safety, and system design.
