Don’t understand how strangers can be so disrespectful to an entire profession dedicated to helping people. It’s not just online that people abuse us. I became active online to bridge the gap between pharmacy and the public but it’s disheartening to see these misconceptions.

Would anyone oppose the UK adopting a USMLE style system to replace the PLAB? Exam fees would then go directly towards the funding for UK Medical Grads to improve education here, whilst simultaneously getting the very best IMGs out there
Dr Dolittle@DocDoLi

Applications to higher training in the last 3 years: 2023: 9,273 UK graduates and 10,402 IMGs applied. 2024: 10,628 UK graduates and 14,868 IMGs applied. 2025: 12,305 UK graduates and 20,803 IMGs applied. Looks sustainable, right?

@ollieburtonmed I would make this clear at the beginning of the shift. Any deviation would be them acting out of supervision.

I’m quite ashamed, as a woman, a gender which understands what it is to be vulnerable to attack, with plenty men intent on harm, that we would rather treat another vulnerable group as a threat, leaving them in harms way, than have empathy & compassion. Many women feel as I do

Woman who's studied barely any anatomy, has never dissected, passed a written nor spot anatomy examination, teaching other people about facial anatomy so that they can shove needles into the public. *wonders why spent an entire term's mornings on head and neck anatomy alone*

@davehartin @LittlePersonDoc Bro she literally mentioned she was called the ED Consultant, how did you read that and think this was an appropriate reply?

@LittlePersonDoc I'm not trying to escalate this any further but I'm going to suggest, respectfully, that as a paediatrician you haven't been fully exposed to EM specialists trying to define what they do versus where they do it over decades and why it matters. A lot. Because it does.

@doctorhelgi @DrFBE @ASiTofficial @Schoolofsurg And as a result thought it was easier for everyone involved if they just assisted. When you’re running around consenting 14 patients you also don’t want to tell them they’ve been cancelled because the list isn’t compatible with you taking a reasonable amount of time to do an op.

@doctorhelgi @DrFBE @ASiTofficial @Schoolofsurg Assisting for 1-2 hernias when you’ve done them before is a good use of time to learn by observation. Repeating that over 14 is not. I’ve been this trainee - probably took one look at the list and thought it was far too busy to accommodate their pace

Today we repaired 14 hernias. Started on time, finished on time. Even stopped briefly for lunch. One operating theatre, no expensive technology. Motivated team, standardised pathway, patients very happy. Annoyingly last list for a while as all extra lists to reduce the number of patients waiting for operations have been stopped for now due to lack of money. #nhs #waitinglists

PA Stephen Nash
PA Stephen Nash@pa_StephenNash·
For each pathology the pa needs to evidence case based discussions, minicex or dops for procedures, accredited courses, and their educational supervisor signs off the level of supervision. Therefore this is the current most robust way of evidencing scope in the field and if utilised properly negates all of the alleged criticisms. It also allows expansion of scope safely as it is done under robust supervision. The supervisor also sets the additional skills and proficiencies to be mapped. Supervisors can also then have a document that clinical supervisors can review when introduced to a new pa. It also enables the restriction of scope to level 2 when the pa joins a new team and is being reviewed by new supervisors. Where the tool really thrives is that it creates an audit trail for supervision that protects the employer by showing that if a pa is negligent it was not the educational supervision that was at fault. This recognises the training as per the curriculum and allows the dynamic management of the individuals scope rather than unnecessarily reduces it because of misinformation and fear mongering over an entire profession. It means employers can utilise the role as designed, show responsible supervision and develop their PA. It will also stop them being held liable for indirectly discriminating against their workforce due to ridiculous statements from colleges suffering from a bullying problem. The use of this tool ends the dispute alongside the relevant map employers guidelines and makes everyone safe as a result - unless of course those with vested interests want to continue punching down on another profession? I'd be happy to have your considered feedback once you review it.

'Content creators' who leave the UK claiming London is 'unsafe' while calling Dubai a crime-free utopia only see their privilege and wealth, never the stark reality... Dubai isn’t some progressive paradise. It’s a place where women can be jailed for reporting rape, punished for pregnancy outside marriage, and trapped in abusive marriages with no legal recourse. These 'creators' aren’t fleeing for safety, they’re fleeing tax bills. They build their wealth in the UK, then take it elsewhere to avoid contributing to the society that enabled their success. There’s nothing progressive about hoarding wealth while refusing to give back, especially when the 'utopia' they run to is built on exploited foreign labour with no rights.

This is why I would like to fill out an advanced directive at the age of 30. No one’s even attempting more than a single cycle of CPR on me, nevermind this mess.

1/ Wild cardiac arrest with #POCUS playing a major role An elderly male was sent from the nursing home in cardiac arrest. Per the prehospital report that we got, the patient was in vfib and refractory to 5 defib attempts. The patient got amio as well as several doses of epi.

@KeepGoinEoin Everyone is missing the point. If he was a nursing home resident he would still have had valid insurance and/ or personal wealth. All of the stuff in the thread is billable, as is his ongoing ICU care etc.

@ariellesophia @elonmusk Use of emotional language to garner sympathy without providing context on the scale of the issue, gee.. never seen that before. 22.3 deaths per 100,000 live births. What, specifically where you working on that was going to stem the tide of those deaths?
