Abdominal mass biopsy – Gastrointestinal Stromal Tumor (GIST): The most common mesenchymal tumor of the GI tract, most often arising in the stomach.
🔬 Morphology: Broad and variable — spindle, epithelioid, or mixed cell types with fibrous to myxoid stroma.
🎯 IHC: CD117+, DOG1+, CD34+.
🧬 Molecular: KIT and PDGFRA mutation analysis for targeted therapy.
Interesting case with a striking myxoid background, showing just how diverse GIST morphology can be.

"Retroperitoneal mass" biopsy – Malakoplakia
🔬 Sheets of foamy histiocytes (von Hansemann cells) with granular eosinophilic cytoplasm and scattered Michaelis–Gutmann bodies — round, basophilic, target-like inclusions within macrophages or extracellularly.
🧫 Special stains: PAS+, von Kossa+, Prussian blue+ (highlighting calcium & iron deposition).
Often associated with chronic E. coli infection and immunocompromised states, secondary to defective phagolysosomal activity with secondary calcifications.

BASALOID SQUAMOUS CELL CARCINOMA OF THE ESOPHAGUS
- Marked male predominance
- Areas of conventional in-situ or invasive SCC are relatively common
- No association with HPV
- Rule out neuroendocrine carcinoma
- Highly aggressive with worse prognosis than conventional
Placenta with toxoplasma placentitis
🧫 Tissue cysts with bradyzoites ± tachyzoites may be found in Wharton’s jelly, amniotic epithelium, and villi.
🔬 Characteristic: diffuse lymphoplasmacytic or granulomatous villitis.
⚠️Often accompanied by hydrops fetalis & placentalis.

Duodenal biopsy in an HIV+ pt with chronic diarrhea:
🔬 Cryptococcus – variably sized (3.5 - 8 μm) encapsulated yeasts with narrow based budding highlighted by GMS & Mucicarmine special stains.
🧫 Cryptosporidium – tiny 2–5µm organisms attached to apical enterocyte surface.
A striking example of dual opportunistic infections in advanced immunosuppression.
