Abdominal mass biopsy – Gastrointestinal Stromal Tumor (GIST): The most common mesenchymal tumor of the GI tract, most often arising in the stomach. 🔬 Morphology: Broad and variable — spindle, epithelioid, or mixed cell types with fibrous to myxoid stroma. 🎯 IHC: CD117+, DOG1+, CD34+. 🧬 Molecular: KIT and PDGFRA mutation analysis for targeted therapy. "Retroperitoneal mass" biopsy – Malakoplakia 🔬 Sheets of foamy histiocytes (von Hansemann cells) with granular eosinophilic cytoplasm and scattered Michaelis–Gutmann bodies — round, basophilic, target-like inclusions within macrophages or extracellularly. 🧫 Special stains: PAS+, von Kossa+, Prussian blue+ (highlighting calcium & iron deposition). Often associated with chronic E. coli infection and immunocompromised states, secondary to defective phagolysosomal activity with secondary calcifications. #PathTwitter #GUPath #SurgPath

Duodenal biopsy in HIV+ (AIDS), CD4 = 2/µL Clinical: Episodes of melena, EGD with inflamed erythematous D2 mucosa. 🔬 Mycobacterium avium intracellulare complex infection: Intestinal mucosa with lamina propria expanded by foamy histiocytes packed with organisms, minimal inflammation.
