I recently hit my 50th peer-reviewed publication. Not that exciting, especially compared to some people I know. But, I’m not a funded researcher. I do patient care and write in downtime. It’s a lot of nights and weekends spent reading, thinking, and writing.

Since it appears impossible to get people to stop saying Poison “Control” Center can we at least call it a Poison “Command” Center? We don’t command or control anything but maybe I could get a cool badge that way. (🤔Poison Consult Center best option now that I think about it)

Every toxicologist screaming into the void over this not-so-great review in @NEJM on NMS. There are many other culprits that need to be ruled out, the authors level/choice of treatment recs are crap, & for the love of god they rec APAP for hyperthermia 🤦🏻‍♂️

a terminally flawed review of neuroleptic malignant syndrome was published in the NEJM yesterday: nejm.org/doi/full/10.10… - no mention of ice-water immersion for hyperthermia - patient in fig. 1 is being cooled with an arctic sun - acetaminophen recommended for hyperthermia

What is the Term for when you do something you don't normally do because of a specific negative outcome, and when you do it, that outcome happens, and its like "yep, still shouldn't do that thing", so you are satisfied despite the negative outcome because you are still right.

If a patient goes into polymorphic point twisting VT that terminates with DCCV and received droperidol within the last few hours, but the QTc is normal on all prior and post ECG's is it torsades?

@GilbertPharmD @TessaPharmD @PharmD_intheED Am I reading this correctly that they essentially took out anion gap from the dx??? if so, I’m ecstatic. Who would’ve thunk it that you must have diabetes with KETO acidosis to be classified DKA. So many premature diagnoses have been made at my shop based on gap alone

I continue to be surprised by the low level of enthusiasm for diuretics in the tx of hyperkalemia. patients with OK kidneys will usually excrete loads of K when treated with agressive diuretics plus IV fluid. this is fundamental, esp in rural setting where HD is delayed (#1/2)

For every PGY1 thats nervous about July 1, theres a PGY2 nervous about no longer being an intern, a PGY3 nervous about being in their last year, and a graduate nervous about being a new attending. And someone with years of experience out there who knows you all will be ok.
