This is Grace Maxwell. Left in 2008, when we told the FAA “the equipage of aircraft with ADS-B In capability will provide an immediate and substantial contribution to safety, especially during operations in and around airports." Right: Grace at the university she was flying back to when she died onboard Flight 5342 on January 29, 2025. I have said many times that our investigation is just the first step. If our recommendations — ALL of our recommendations — are not fully implemented or they are watered-down to the point where they are no longer our recommendations, more lives will be lost. Grace’s death was preventable! She should be with her family today.

Today would have been Melissa’s 30th birthday. She died on January 29, 2025, onboard Flight 5342. She should be here tonight celebrating with her fiancée Ben, her family, and her friends. Instead, they are relentlessly fighting for safety change in her honor. This was preventable, and we issued 74 findings and 50 recommendations that, if fully implemented, would’ve saved her life… will save other lives.

This is #safety leadership! @FAANews Bryan Bedford at Verticon 2026: “This idea of ADSB or electronic conspicuity, we need to broadcast if we’re going to tighten up separation standards. We have to have a lot higher fidelity on where aircraft are at all times in the airspace, and I don’t know why this has proven to be so difficult given the affordability and availability of portable transponder devices. But, again, in the hear and now, we want absolute safety and to do that we need greater fidelity on where all vehicles are in the airspace at all times.” Well said! 👏👏👏 youtu.be/Ondl5FrUmCA

This is Anthony Parente sitting beside his fiancée. They were supposed to be planning a wedding. Instead, she had to plan a funeral. A life together gone in an instant. In 2008, the @NTSB recommended ADS-B In technology to help prevent midair collisions.

After Flight 5342 crashed into the Potomac in January 2025, NDMS’s DMORT and VIC deployed. DMORT worked side by side with DC's Chief Medical Examiner to process remains. VIC worked with the Family Assistance Center, talking to families and gathering information on unidentified victims. Both teams played a key role in helping the families of the 67 victims begin to find closure. In honor of their outstanding service, @dcfireemsfound and Families of Flight 5342 presented them with a plaque and challenge coin.

Great meeting with @SecArmy Driscoll this week. We discussed the @NTSB #safety recommendations we issued in the final report of the midair collision near DCA. The @USArmy was an excellent partner throughout our investigation; they’ve done tremendous work to implement our recommendations.
