The government has been de-valuing physician care for decades. Physician payment from 2005 -> 2021: -2.1% Non-physician payment during the same period: +206.5% This is the problem with administrative pricing. It follows the whims of congress and CMS bureaucrats without any relation to market forces. Have non physician practitioners truly become 200% more valuable since 2005? Have doctors become less valuable? Administrative pricing is the only alternative to market-based approaches. It's clear what the administrators think of our value. @cscla @MayaBabuMD @DrBrian4Health @JPGK_MD @joshuarosenowMD @neurosurgery @anish_koka journals.sagepub.com/doi/10.1177/00…

Red Flag = No MOC?? Did you know that ABMS advocates for MOC via NAMSS (National Association of Medical Staff Services) to every medical staff office in the United States? FTC, DOJ and HHS are listening, ALL PHYSICIANS comment now: justice.gov/atr/webform/su…

I personally use Vanguard Money Market Fund. It's a mutual fund that invests in short-term treasury and government obligations (not FDIC insured)

I researched FDIC insured high-yield savings accounts that pay up to 6%. Here is the list:

@AaronGoodman33 @doctorwes What’s crazy is my father who hasn’t touched his IM boards for 20 years was suddenly reported not certified in anything despite completing Cardiology boards 2 years ago. No warning or anything. Just reported to hospitals as he is no longer certified in anything.

Great to work on the largest CT simulation to date of redo #TAVR feasibility in patients following S3 valves with @GilbertTangMD @ParasuramMD @DoctorKini and team @MountSinaiHeart. Implant depth and commissural alignment - both affect #TAV-in-TAV options
Dr. Gilbert Tang@GilbertTangMD

Just out! Our @CircIntv redo #TAVR CT simulation with S3 >1900 pts. Higher implant ≠ better in feasibility. @DrAnoop_Koshy @ParasuramMD @KHERA_MD @DoctorKini @LucySafi @MountSinaiHeart @MountSinaiNYC @IcahnMountSinai ahajournals.org/doi/10.1161/CI…

Taxpayers pay $15+ Billion annually for physician residency training so an unregulated nonprofit (ABMS) can remove "Certified" status for not purchasing their unproven proprietary continuing education product (MOC). Join the fight and email ACGME now bit.ly/4bRJKKP

Here is a nice schematic from our first @aahks health policy fellowship meeting outlining what everyone is discussing - reimbursements to hospitals have a law factoring in inflationary benefit while physicians do not

"I have buildup of cholesterol plaque in the heart arteries? What can I do to shrink this buildup?" Best evidence for atherosclerosis regression: get LDL-C to very low levels GLAGOV trial of PCSK9i when added on top of statin LDL-C levels very low (like in 30s, 40s) (x-axis) led to shrinking of plaque (y-axis)

@CMichaelGibson I wonder if it’s because many fellows will pursue further training. You take your General Cardiology boards during your Interventional training or EP training or Advanced Heart failure which is quite intense. Why aren’t boards offered just prior to you completing training?

@PoojaJagadishMD @DrMarthaGulati @drscottyk I’ve never seen it covered. But I find it helpful even if I’m planning statins. In the event they don’t tolerate it or if they have extremely high calcium score and I would want more aggressive LDL control, having documented CAD helps with getting a PCSK9 approved later.

When did obtaining a Coronary Calcium Score in patients with familial hypercholesterolemia become an "Investigational / Noncovered Study" by insurance plans? It's a low-dose, quick test that can help risk-stratify patients who need statin optimization! @DrMarthaGulati @drscottyk

@emily_fri IMHO, burnout is directly proportional to loss of control. No amount of lectures to anyone is going to fix this until physicians regain a reasonable amount of autonomy over how they practice medicine.
