My Nan died on Saturday peacefully, pain free, unbelievably medication free & it appeared to be on her terms, in her own time. I will be forever grateful for the support from my NHS family & the care from the NH for those 2 short weeks. My brain isn’t ready yet for the quiet.💖

@djbeckett Themes: Long Term conditions/Frailty/Mental Health/Complex comorbidity/Approaching the end of life? Anticipatory Care DES and Proactive Care DES and Shared Care Records are the tools for the new ICSs to tackle this very issue. Won’t solve things overnight but a good start

Another (very) busy weekend on-call. Striking how many patients are falling between gaps. Either between hospital specialities, or between primary care and secondary care, or health and social care. The problem with these gaps is they’re nobody’s problem to fix. Really tricky.

@lucypgeridoc It’s madness. So much for personalised care. How do we reset in an NHS that is pathway driven and guideline orientated? How to we enable the silent masses to rebel? How do we encourage our hard pressed colleagues to engage with people not processes?

@lucypgeridoc from my father today- 84 yesterday and wouldn’t have cake as had been told he needed another Hba1C following last weeks abnormal one! He’s 84! Normal bmi and previously normal hba1c! Finally persuaded him he could have some birthday cake. We need to stop the madness @nkl_17
