Your installers came to my mum's house on Monday. Her gas boiler has not worked since they turned it off. You are refusing to sort the issue you have caused. You finally agreed to come in another 6 days, leaving an elderly lady without heat or hot water for over a week. @currys

Anyone lost patience with @sonos and sold their speakers (if anyone would buy them) and switched to a different system? Seems so wasteful but the system borders on unusable much of the time. What did you choose instead?

@dr_woodall @Paulio1984 @BenAllenGP I haven't seen those comments, though I'm sure you're right. This isn't about the GP taking responsibility, though. It's about how care can be delivered closer to home and results acted upon by appropriate person - a truly integrated system.

@BenAllenGP No, it doesn't. These are exactly the sorts of discussions we ought to be having. Why can't this sort of pathway be achieved by partners agreeing how resource flows appropriately to enable it?

@keester76 Agree, it makes sense to be done in community. It just needs the work to be funded and contractual.
30p/ day to look after each patient doesn't enable that.

@Drjimwood65 @BenAllenGP Exactly this. Capacity and funding would be appropriately resourced to do this in a truly integrated system.

@keester76 @BenAllenGP The results need processing by a GP though. Having said that, it totally makes sense for it to be done in GP but the capacity and funding isn’t there for it. (Also ECGs aren’t available in a lot of surgeries as ECGs aren’t in the core contract and are unfunded)

@adje__ @BenAllenGP Exactly. We don't have a truly integrated system that allows this sort of pathway to be achieved with resources directed to the right places to do it yet.

@keester76 @BenAllenGP Because it ain't a truly integrated system and GP gets less only 7% of total NHS funding. Unless secondary care is prepared to take a pretty big pay cut, then it needs to step up and start actually fulfilling its contractual duties.

@dr_woodall @BenAllenGP Well, no, which is why I specified 'a truly integrated system'. A truly integrated system that put patient journies ahead of funding streams would ensure community based teams received resource it needed to do it.
